Has anyone seen Dr Amit Allahabadia, at Sheffield and London.

I’m m trying to find an endo from the Thyroid Uk list and wondered if anyone is a patient or has had a consultation with him.

BEFORE considering booking any consultation you need to retest vitamin levels

8 months ago vitamin D was severely deficient, B12, folate and ferritin all inadequate

Have you had increase in levothyroxine since then or are you still on 100mcg

Which brand of levothyroxine are you currently taking

Do you always get same brand levothyroxine at each prescription

ESSENTIAL to get all four vitamins retested and at optimal levels

Vitamin D at least over 80nmol and between 100-125nmol may be better

Serum B12 at least over 500

Active B12 at least over 70

Folate and ferritin at least half way through range

Ft4 and Ft3 at least 50-70% through range

Always test thyroid levels early morning, ideally just before 9am and last dose levothyroxine 24 hours before test

Come back with new post once you get results

I replied earlier but can’t find it now, so apologies if I post this twice.

Vit D - followed advice on here and the last test was much better. Still on vit d/k2.

Ferritin is low but I think I need an iron panel before starting iron. I’m vegetarian so may answer why I’m low in some areas.

I have a test for T3 (includes T4 and TSH)- have been waiting to avoid the Christmas post so can do that on Monday.

Vit B12, serum is below 500 (no active given). The options online are plentiful, are there any tips for choosing the most appropriate?

I haven’t tried gluten free, would struggle with that to be honest, but am looking at testing to see if I should pursue it.

Not doing so well with the endo hunt so I will make another request for people to PM me. I had a helpful response from my last one.

You can’t take iron supplements until got full iron panel test

But as a vegetarian it’s highly likely iron and ferritin are low

Look at increasing iron rich foods in diet 

Eating iron rich foods like liver or liver pate once a week plus other red meat, pumpkin seeds and dark chocolate, plus daily orange juice or other vitamin C rich drink can help improve iron absorption

This is interesting because I have noticed that many patients with Hashimoto’s disease and hypothyroidism, start to feel worse when their ferritin drops below 80 and usually there is hair loss when it drops below 50.

Never supplement iron without doing full iron panel test for anaemia first and retest 3-4 times a year if self supplementing. It’s possible to have low ferritin but high iron

We have received further information the lab about ferritin reference ranges. They confirm that they are sex dependent up to the age of 60, then beyond the age of 60 the reference range is the same for both sexes: 

Males 16-60: 30-400 ug/L

Female's: 16-60: 30-150

Both >60: 30-650 

The lower limit of 30 ug/L is in accordance with the updated NICE guidance and the upper limits are in accordance with guidance from the Association of Clinical Biochemists. ‘

Have you been working on improving low /deficient vitamin D, folate and B12 over last 8 months?

if not you should retest NOW

There’s no point seeing any thyroid specialist until vitamins are considerably better

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a B12 supplement as well as a B Complex (to balance all the B vitamins)

once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to stop the B12 and just carry on with the B Complex.

But as vegetarian likely to need extra separate B12 2-3 times a week

Note that improving folate when B12 is very low is not a good idea. Taking folate before B12 is good enough can lead to severe neurological problems.

B vitamins best taken after breakfast

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) 

Thorne currently difficult to find at reasonable price, should be around £20 

If you want to try a different brand in the meantime, one with virtually identical doses of the ingredients, and bioavailable too, then take a look at Vitablossom Liposomal B Complex. Amazon sometimes has it branded Vitablossom but it's also available there branded as Yipmai, it's the same supplement
