New blood results: please help interpret!

Hi All,

Since I last posted, i haven’t changed any meds (still on 125mg of levo mercury pharma brand, taken at night) I tried to take the vitamin b complex recommended on here, but it gave me extreme itching sensations- so i stopped this. (That was 2 weeks ago)

I decided to get an advanced thyroid panel before going to see a new endo next week. As I posted on here, my last endo was not very helpful, practically gave up on me and referred me to an immunologist. I’m not against seeing an immunologist but i’d rather find a capable endo first who listens to my symptoms (which are very hypo!)

Background: Hashis, still suffering from low energy, brain fog, memory loss, dry skin. Been told on here that I am not converting as well as I should- is this still the case?

My tsh has creeped back up a bit closer to 1 again too..

Results thyroid panel 27 may 2022

Tsh 0.93 mIU/L (0.27-4.2)

Free t3: 4.4 pmol/L (3.1-6.8)

Free thyroxine: 21.8 pmol/L (12-22)

Folate serum: 4.2 ug/L (>2.9)

Vitamin b12 active: 105 pmol/L (25.1-165)

Vitamin d: 139 nmol/L (50-200)

Ferritin: 146 ug/L (13-150)

CRP HS: 0.8 mg/l (0-5)

Thyroglobulin antibodies: 331 IU/ml (0-115)

Thyroid peroxidase antibodies: 210 IU/ml (0-34)

Please help interpret!

Assuming you did your test as we advise, ie

* No later than 9am

* Water only before test

* Last dose of Levo 24 hours before test

* No biotin in any supplement for 3-7 days before test

Tsh 0.93 mIU/L (0.27-4.2)

Free t3: 4.4 pmol/L (3.1-6.8) = 35.14% through range

Free thyroxine: 21.8 pmol/L (12-22) = 98% through range

So yes, conversion is still very poor. Low T3 is probably causing your symptoms.

Folate serum: 4.2 ug/L (>2.9)

Very low.

Vitamin b12 active: 105 pmol/L (25.1-165)

This is a good result.

Vitamin d: 139 nmol/L (50-200)

This is a good result.

Ferritin: 146 ug/L (13-150)

Are you supplementing? You don't really want this going any higher.

CRP HS: 0.8 mg/l (0-5)

Good result, the lower the better with CRP.

Do you take any supplements for B12 and Vit D?

Hi SeasideSusie, thanks for your reply. I don’t take any supplements. As i mentioned, i tried to take the b complex recommended to me but it made me extremely itchy so I stopped taking this two weeks ago! Maybe there is something I can take just for folate?

My ferritin levels are always high! Ive been told that this is nothing to worry about.

Interesting to see that the conversion is still poor. My previous endo did not seem to think it was poor conversion (hence why I am hoping the new one i’m trying next week will consider this!)

So you don't know what it was in the B Complex that caused the itching, it could be one of the B vitamins or it could be one of the fillers.

You could try just a methylfolate supplement but it's better to try and keep all of the B vitamins balanced.

You could look at supplements with no fillers, Health Leads is one UK company that says they use no fillers:

It's not a recommendation as I've not used them, I just know they are additive free and I'm sure there are other companies that ala make additive free supplements.

High B12 can be deceiving especially with low folate

Either try different vitamin B complex or a separate folate

Have you had coeliac blood test done

Are you on strictly gluten free diet or dairy free

Hello there :

The accepted conversion ratio when taking T4 only is said to be 1 / 3.50 - 4.50 - T3/ T4 - with most people feeling at their best when they come in at around 4 or under.

So, to see how well you convert T4 into T3 you just divide your T3 into your T4 and I'm getting yours coming in at 4.95 so, yes, your conversion is not good.

No thyroid hormone replacement work well until ferritin, folate, B12 and vitamin D are up and maintained at optimal levels, and conversion can also be compromised by inflammation, and any physiological stress ( emotional or physical ) depression, dieting and ageing.

Since you have Hashimoto's AI thyroid disease you are liable to experience ' swings ' in your T3 and T4 levels as the gland becomes progressively disabled by this auto immune disease.

It's suggested that you get checked out for Celiac Disease and looking at food intolerances and healing the gut to enable maximum absorption of thyroid hormones and I read the research and work of Dr Izabella Wentz has helped many people to deal with this AI disease.
