Four weeks following diagnosis - help with interpreting medichecks blood test please

Hiya, I am four weeks in, after being diagnosed with an underactive thyroid. I took a medichecks blood test, following advice in here, as my gp test was very basic and I know my mum can't absorb b12. These are the results I got today and I would love some help interpreting them.

I must admit I feel loads better since starting on levo (100mg) but still feel a bit all over the place. My legs feel really weak and although I seem to have more energy, it still crashes regularly. I don't fully understand how everything works regarding t3 and t4, but I am definitely learning. 😊 Thank you all for being so knowledgeable

CRP HS 3.75 mg/L (Range 0-5)

Ferritin 59.3 ug/L (Range 13-150)

Folate - Serum 6.25 ug/L (3.89 - 19.45)

Vitamin B12 Active 37.7 pmol/L (Range 37.5 - 188)

Vitamin D 14 nmol/L (50-200)

TSH 3.28 mU/L (Range 0.27 - 4.2)

Free T3 5.00 pmol/L (Range 3.1 - 6.8)

Free Thyroxine 21.3 pmol/L (Range 12-22)

Thyroglobin Antibodies 95 IU/mL (<115)

Thyroid Peroxidase Antibodies 76 IU/mL (<34)

Take these results to your GP

B12 is extremely low. Do NOT start any B vitamins. You need testing for Pernicious Anaemia, especially as your Mum has similar issue

Is she diagnosed as actually having PA?

Vitamin D is also extremely deficient

You will need LOADING dose vitamin D. That’s 300,000iu vitamin D in total over 6-8 weeks and vitamin D levels retested at end of prescription. You will definitely need ongoing vitamin D supplement every day

Have you had coeliac blood test done yet

If not, assuming you are not already on strictly gluten free diet, you need coeliac blood test done

Likely to need B12 injections

Suggest you join PAS healthunlocked too and ask their advice

B12 injections usually start with several injections over first few weeks, then gap is typically every 2-3 months, but can be more frequent if necessary

NHS Guidelines on dose vitamin D required

ouh.nhs.uk/osteoporosis/use...

GP will often only prescribe to bring vitamin D levels to 50nmol Some CCG areas will prescribe to bring levels to 75nmol or even 80nmol

leedsformulary.nhs.uk/docs/...

GP should advise on self supplementing when levels are over 50nmol, but under 75nmol (but they rarely do)

But with Hashimoto’s, improving to around 80nmol or 100nmol by self supplementing may be better

Once you Improve level, you will need on going maintenance dose to keep it there.

Test twice yearly via NHS private testing service when supplementing

There’s a version made that also contains vitamin K2 Mk7. One spray = 1000iu

Thank you for this... I will ring the doctor tomorrow.

I'm not sure if my mum had a diagnosis, but she was on injections for a few years and felt quite ill without them.

Just say to GP your mum has PA and on B12 injections

Come back with new post once you have seen GP and update on what they have done and prescribed
