Not currently got diagnosis but dad had hypothyroidism.

TSH 3.75 now was 5.95 in February and 1.95 in December (normal range 0.35-4.94)

T4 - 10.4 now and was 11.5 in February (normal range 9.0-19.0)

Feel terrible but doctors are saying they are normal except the tsh 5.95 in February

I had my test in afternoon (as only appointment I can get at my gps)

Vitamin d , 26 (normal range 50-)

Folate - 5.1 (normal 3.1-19.9)

Ferritin - 36 (normal 15-200)

B12 - 301 (normal 150-620)

They said they were doing antibodies but doesn’t look like they have unless they have yet to come back ,

To make it possible for us to interpret your results accurately, we need to know if you have a diagnosed thyroid condition and if so are you on any thyroid medication.

Also we need the reference ranges that come with your results, ranges vary from lab to lab so we need the ones that your lab uses, eg. FT4: 10.4 (9-19).

Once we have this information we will be in a better position to help you.

Do you have a diagnosis of hypothyroidism and on levothyroxine? Results are interpreted differently before and after starting Levo.

Did you test as we advise:

* No later than 9am

* Nothing to eat or drink except water before the test

* No biotin or any supplement containing Biotin for 3-7 days before the test (eg B Complex or hair/nail supplement

* If taking Levo, did you take your last dose 24 hours before the test

ALWAYS test thyroid levels early morning, ideally before 9am

For full Thyroid evaluation you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested.

Very important to test vitamin D, folate, ferritin and B12

What vitamin supplements are you currently taking

Low vitamin levels are extremely common, especially with autoimmune thyroid disease (Hashimoto’s or Ord’s thyroiditis)

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis. Both are autoimmune and generally called Hashimoto’s.

In U.K. medics never call it Hashimoto’s, just autoimmune thyroid disease (and they usually ignore the autoimmune aspect)

Recommended on here that all thyroid blood tests early morning, ideally before 9am last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

If TPO or TG thyroid antibodies are high this is usually due to Hashimoto’s (commonly known in UK as autoimmune thyroid disease). Ord’s is autoimmune without goitre.

About 90% of all primary hypothyroidism in Uk is due to autoimmune thyroid disease .

These are all too low and need improving

GP is obligated to prescribe vitamin D as result is below 50

However you may prefer to self supplement at higher dose to increase levels quicker

GP should prescribe 1600iu everyday for 6 months

GP will often only prescribe to bring vitamin D levels to 50nmol. Some CCG areas will prescribe to bring levels to 75nmol or even 80nmol

Low folate and low B12

Aiming for folate at least half way through range

B12 at least over 500

As they are within range, NHS won’t treat. Down to you to self supplement to improve

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid) may be beneficial.

This can help keep all B vitamins in balance and will help improve B12 levels too

Many Hashimoto’s patients have MTHFR gene variation and can have trouble processing folic acid.

B vitamins best taken after breakfast

Thorne Basic B or Jarrow B Right are recommended options that contains folate, but both are large capsules. (You can tip powder out if can’t swallow capsule)

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a B12 supplement as well as a B Complex (to balance all the B vitamins) initially for first 2-4 months, then once your serum B12 is over 500 (or Active B12 level has reached 70), stop the B12 and just carry on with the B Complex.

In all people, a serum ferritin level of less than 30 micrograms/L confirms the diagnosis of iron deficiency

Look at increasing iron rich foods in diet

Eating iron rich foods like liver or liver pate once a week plus other red meat, pumpkin seeds and dark chocolate, plus daily orange juice or other vitamin C rich drink can help improve iron absorption

This is interesting because I have noticed that many patients with Hashimoto’s disease and hypothyroidism, start to feel worse when their ferritin drops below 80 and usually there is hair loss when it drops below 50.
