I had my latest blood test come back.

Tsh..2.00 (0.27- 4.2)

T3..3.8 (3.1 -6.8)

T4..9.5 (12- 22)

At the moment I am on 50mcg

Thyroxine and 15 mcg of thybon Henning. My Endo wants to put my t3 up to 20mcg and leaving my thyroxine dose as it is.

I dont understand why.Could anyone explain this.

Because your endo hasn't got a clue?

2 months ago you were on 1/2 grain NDT and 15mcg T3.

How come the change to 50mcg Levo instead of the NDT?

Are you paying for this? If so I think you're wasting your money on an endo who really doesn't know what he's doing.

What I would do in your position is stick to one thing, let's say you are going to stay on Levo plus T3. So you want your TSH down to below 1 - when taking NDT or T3 TSH is usually very low or suppressed anyway.

Unless you're on T3 only one would expect to see FT4 somewhere in range when on combination hormone replacement. So I would increase Levo to bring your TSH down and get your FT4 up. This should also increase your FT3 depending on how much natural conversion you have.

I would add 25mcg Levo now then wait 8 weeks and retest. Don't do anymore chopping and changing, just see how it goes with the increased Levo plus T3.

Are you nutrient levels optimal -Vit D, B12, Folate, Ferritin?

Hi seasidesusie I decided to come off NDT altogether as my levels would,nt stabilise. Yes I am paying for my t3, but my doctor prescribes the thyroxine.But he wont prescribe it until he gets the ok from my Endo regarding what dose I should be taking ,so I'm in a bit of a dilemma. Can you get palpitations if your dose is to low?Last test I had done for my vit levels were a while ago,I intend to get those checked soon.I usually go with medichecks,given up asking at the doctors .

Yes. Palpitations can be a symptom of hypothyroidism so if you are undermedicated then this could be the cause.

Because your Ft3 result is low at 3.8 in range. Your TSH is a bit high for someone on replacement hormones.You could raise ft3 by increasing levo but this would depend on how well you convert.

The endo has decided that the liothyronine dose will increase this time.

Many people on levothyroxine plus T3 need BOTH Ft4 and Ft3 roughly 50-60% through range

As your Ft4 is currently below range recommend getting levothyroxine dose increase to 75mcg FIRST

This will improve both Ft4 and Ft3 levels

Retest 6-8 weeks after each dose change or brand change in levothyroxine

Which brand of levothyroxine are you currently taking

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water .

Last dose of Levothyroxine 24 hours prior to blood test. (taking delayed dose immediately after blood draw).

Do you normally split the T3 dose into 2 or 3 smaller doses

Day before test split dose of T3 into 3 smaller doses, taking at approx 8 hour intervals, with last dose 8-12 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Is this how you do your tests?

Essential to regularly retest vitamin D, folate, ferritin and B12

Hi Slowdragon, I take my 50mcg thyroxine and 10mcg in the morning about 7.30 then I take my 5mcg t3 at 4.30 afternoon.My vit levels were taken in June.Vitamin D...86

Ferritin 80 (11-307)

B12. 246 (120.00 -625)

Folate 6.5..

I dont feel well on 50mcg thyroxine.I emailed my Endo and told him I feel that perhaps my dose should be raised but he

Insists on putting my t3 up,I cant understand it.As you said earlier get my t4 up and deal with the t3 later.I intend to

tell him I dont want my t3 put up I want the t4 raised.If he doesn't do that I dont know what to do,my doctor will only prescribed what the Endo tells him to.

Your B12 and folate are far too low as well

Low vitamins common when under medicated

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid) may be beneficial.

This can help keep all B vitamins in balance and will help improve B12 levels too

B vitamins best taken after breakfast

Thorne Basic B is an option that contain folate, but is large capsule. You can tip powder out if can’t swallow capsule

Igennus Super B is good quality and cheap vitamin B complex. Contains folate. Full dose is two tablets per day. Many/most people may only need one tablet per day. Certainly only start on one per day (or even half tablet per day for first couple of weeks)

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a B12 supplement as well as a B Complex (to balance all the B vitamins) initially for first 2-4 months, then once your serum B12 is over 500 (or Active B12 level has reached 70), stop the B12 and just carry on with the B Complex.

Have you already been taking any B12 supplements

You really need to take separate B12 as well as B complex, until serum B12 is over 500

I was taking vitamin b12 until a couple of weeks ago I had run out.I was taking Health Leads 500ug,one a day,which actually brought my levels up to where they are now,it was a lot lower.No I'm not vegan or vegetarian.So do I take the vitamin b12 and the vitamin b complex together?

Tell your endocrinologist you wish to try increasing levothyroxine by 25mcg

An endocrinologist can not dictate, they should advise, and listen to patient’s wishes

If endocrinologist won’t agree you need new one
