Thyroid levels help

Hi i just wanted to check if this is an optimal level after my thyroxine was reduced because I was getting overactive symptoms thanks

How are you feel now?

Was the retest 6 weeks later?

Was this timing of test as recommended? Early morning draw, fast overnight, delay replacement until after draw?

Optimal is individual, your optimal will be different to someone else’s.

Yes test was done as reccomended This was the 2nd retest 3 months after reduction first TSH was 0.9

I still feel a little bit agitated and heart rate sometimes is a bit fast but not sure if that is anxiety. I wondered if my T4 was still a little high

FT4: 18.3 pmol/l (Range 12 - 22) 63.00%

FT3: 4.3 pmol/l (Range 3.1 - 6.8) 32.43%

Your FT4 isn’t too high in range, most would be ok or prefer a little higher, but your FT3 is quite low for most & many say they feel better with FT3 over 50%.

Has your doctor tested key nutrients recently? Folate,ferritin, B12 & vitamin D?

Optimal key nutrients can improve T4:T3 conversion. You might feel less symtoms with a more balanced FT4:FT3.

Many test privately, with home fingerprick tests as often labs / doctors will not tests everything.

Test was taken early morning before meds and no food. Dose is 75mg per day and had diagnosed hypo for 20 years and i'm 40.

Free T4 (fT4) 18.3 pmol/L (12 - 22) 

Ft4 63.0% through range

Free T3 (fT3) 4.3 pmol/L (3.1 - 6.8)

Ft3 only 32.4% through range

Which brand of Levo are you taking

Do you always get same brand levothyroxine at each prescription

Very poor conversion rate of Ft4 (levothyroxine) to Ft3 (active hormone) suggests low vitamin levels likely

You need to test vitamin D, folate, B12 and ferritin

What vitamin supplements do you currently take

also is your hypothyroidism autoimmune?

Low vitamin levels are extremely common when hypothyroid, especially with autoimmune thyroid disease

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high TPO and/or high TG thyroid antibodies

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis.

Both are autoimmune and generally called Hashimoto’s.

Significant minority of Hashimoto’s patients only have high TG antibodies (thyroglobulin)

NHS only tests TG antibodies if TPO are high

20% of autoimmune thyroid patients never have high thyroid antibodies and ultrasound scan of thyroid can get diagnosis

In U.K. medics hardly ever refer to autoimmune thyroid disease as Hashimoto’s (or Ord’s thyroiditis)

Recommended that all thyroid blood tests early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Private tests are available as NHS currently rarely tests Ft3 or all relevant vitamins

If you normally take levothyroxine at bedtime/in night ...adjust timings as follows prior to blood test

If testing Monday morning, delay Saturday evening dose levothyroxine until Sunday morning. Delay Sunday evening dose levothyroxine until after blood test on Monday morning. Take Monday evening dose levothyroxine as per normal

I am taking accord for 50mg and Wockhardt 25mg I always insist the same brands and take my thyroxine mornings. I don't eat breakfast so it's just with water Last month Ferritin was 46 I usually take iron supplement but I've had to have a break because of my stomach being so bad. I take tranexamic acid for my period to help.

Vit D was 81

B12 was 482

I cant remember folate but I remember checking and it was a good result.

I have autoimmune without goitre

Actually I've just checked my system online and think it's been over a year since folate was checked. My GP is good so I can ask for that
