Is your K2 a tablet, capsule or oil based liquid drops. If a tablet or capsule are you taking it with some fat, Vit K is a fat soluble vitamin and needs fat to be absorbed.
B12, Vitamin D3, Magnesium L-threonate and Iron at 12pm
You need to take your iron 2 hours away from any other supplement (and medication) as it affects their absorption.
D3 - same as K2, needs fat to be absorbed.
Also should I take T3 at the same time as T4?
There's no problem with taking them at the same time. You might want to split your T3 into 2 or 3 doses, some people do, others take it all at once. I split mine into two, I take 12.5mcg early morning with my Levo and 6.25mcg at 5pm.
Should I reduce my Levo to 75mcg and take 18mcg of T3. In line with how I dosed on NDT?
You might find that you can't swap like that, but it's a good place to start, retest after 6-8 weeks and see what your levels are like and how you feel, then you might find you need to adjust something. Only change one thing at a time, never both Levo and T3 together or you wont know which you benefit from.
K2 is tablet and I take it with full fat yogurt. Vit D is olive oil capsule.
So willl change Iron to 11am and others to 1pm.

The 20mcg (and 5mcg) tablets will be on prescription.
I self source and mine are 25mcg so I take half a tablet in the morning and 1/4 of a tablet in the afternoon.

I mentioned above that what you suggested was a good starting point - 75mcg Levo plus 18mcg of Levo (but would be 18.75 if you have 25mcg tablets). If you were swapping straight from NDT to Levo plus T3 it would be easier, but as you've been on just 150mcg Levo it might be better to maybe drop the Levo to 125 for a week or two then add in 1/4 of a tablet of T3 (6.25mcg) for a couple of weeks then maybe add another 1/4 of a tablet and see how things are. It takes a lot of tweaking and you can't really rush it.
