Should I increase medication?

Please can anyone advise if I should increase medication . I take 100/125 mcg thyroxine alternate days and 6.25mcg T3 daily . Meds haven’t changed for about 18 mths but I now have all of the hypo symptoms back ... hair loss, constipation, weight gain etc etc. My blood test results today are

TSH. 0.03 (0.27-4.2)

T3 4.6 (3.1-6.8)

T4 19.8 (12-22)

B12 done in Dec was >150 (37.5-187.5)

Iron was also done and in range but I can’t find results at present time

Bloods were taken 1st thing in morning

I’m not sure if I should increase T3 or T4 or both

I look forward to your replies . Thank you

Was last dose levothyroxine 24 hours before test

That’s an extremely small dose T3. Are you self medicating T3?

Many people on T3 will need 1/4 tablet (6.25mcg) 2 or 3 times per day

2 doses at approx 12 hour intervals

Or

3 doses at roughly equal 8 hour intervals

Day before test did you take T3 dose approx 8-12 hours before test?

When were vitamin D, folate and B12 last tested

Thank you for your reply , yes last dose Levo was 24 hrs before . T3 was 12 hrs before and yes I am self medicating. Last B 12 test was Dec , I have listed results in my post. I take Vitamin D, magnesium and zinc daily and have lost my last iron results which was also tested at end of 2020 but was in range.

Suggest you add 2nd 6.25mcg (1/4 tablet ) T3 late afternoon

Retest in 6-8 weeks

You might want to consider reducing levothyroxine dose very slightly at same time

Perhaps reduce to 100mcg 6 days week and 125mcg 1 x week

Did you get a reverse T3 lab done? I bet it’s high from to much T4.
