Help for my daughter

Hi Everyone,

I would really appreciate your advice re my daughters blood test result.

She is on a strange combination, but feels that it has been the better option for her. She has tried:

Just levothyroxine

Levothyroxine with added T3 (Tried numerous dosages)

Just Nature Throid

Nature Throid with added T3

Nature Throid with levothyroxine (This is what she is happiest on (Even though not perfect, she feels better than all of the above)

She is taking 125 mcg of levothyryoxine together with 1.5 grains of Nature Throid.

Her results are from a recent blood test: (Dr does not test T3)

Free T4 13.3 pmol/L (12.0-22.00)

TSH 10.06 mU/L (0.27-4.20)

She has low Vit D, Low Ferritin and Low folate which she will take supplements for.

My daughter has gained a lot of weight, is fatigued, breathless etc. We can sort her iron out etc, but would really like your expertise help to sort her thyroid medication and dosage. should she increase her Nature Throid or Thyroxine or can you recommend other thyroid medication?

The tsh suggests that she is undermedicated. However we really need a Ft3 result.When on a combo treatment Ft3 really must be tested along with ft4. Many on here resort to postal testing as their gp will not do ft3.

If your daughter wishes to balance her thyroid hormones and fine tune doses she needs proper blood results.

How is she taking her levothyroxine

Her TSH is very high for someone on levothyroxine and NDT

Exactly What vitamin supplements is she currently taking

We need OPTIMAL Vitamin levels for levothyroxine and NDT, T3 to work well

Does she have Hashimoto’s

Is she on strictly gluten free diet

For full Thyroid evaluation she needs TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested. Also EXTREMELY important to test vitamin D, folate, ferritin and B12

Low vitamin levels are extremely common, especially if you have autoimmune thyroid disease (Hashimoto's) diagnosed by raised Thyroid antibodies

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water .

Last dose of Levothyroxine 24 hours prior to blood test. (taking delayed dose immediately after blood draw).

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

If/when also on T3, or NDT make sure to take last third or quarter of daily dose 8-12 hours prior to test, even if this means adjusting time or splitting of dose day before test

Is this how you do your tests?

Private tests are available as NHS currently rarely tests Ft3 or thyroid antibodies or all relevant vitamins

How old is she?

If taking any supplements that contain biotin (like vitamin B complex) it’s important to stop these a week before ALL BLOOD TESTS as biotin can falsely affect test results

If testing ferritin levels need to stop iron supplements a week before testing to get true ferritin result

Exactly what vitamin supplements is she taking

When were vitamin D, folate and ferritin last tested

What were results and ranges

Does she have Hashimoto’s

Is she on strictly gluten free diet

Looking at previous posts....she has Pernicious Anaemia

This is autoimmune, so assuming her hypothyroidism is also autoimmune thyroid disease - Hashimoto’s

How frequently are her B12 injections

As she has B12 injections it’s recommended on here also to supplement a good quality daily vitamin B complex, one with folate in (not folic acid) may be beneficial.

This can help keep all B vitamins in balance and will help improve B12 levels between injections too

Don’t start vitamin B complex until after done private testing

Many Hashimoto’s patients have MTHFR gene variation and can have trouble processing folic acid.

B vitamins best taken after breakfast

Igennus Super B is good quality and cheap vitamin B complex. Contains folate. Full dose is two tablets per day. Many/most people may only need one tablet per day. Certainly only start on one per day (or even half tablet per day for first couple of weeks)

Or Thorne Basic B is another option that contain folate, but is large capsule

If taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results
