Thyroid function test

I had an overactive thyroid - which was knocked out unfortunately with RAI treatment about 20 years ago.

I had a blood test recently:

Thyroid function test (X77Wg)

Serum TSH level (XaELV) <0.01 miu/L (0.3-4.2) Below low reference limit

Serum free T4 level (XaERr) 19.1 pmol/L (9-23)

Thyroid function test. Report. Borderline.

Could you please tell me if this is borderline or not - I am on 125mg of levothyroxine.

How do you feel?

Just testing TSH and Ft4 is completely inadequate

Do you always get same brand of levothyroxine?

What vitamin supplements are you currently taking?

For full Thyroid evaluation you need TSH, FT4 and FT3 tested. Also EXTREMELY important to test vitamin D, folate, ferritin and B12

Low vitamin levels are extremely common, especially if you have low Ft3

Low vitamin levels can significantly reduce conversion of levothyroxine into active Ft3

If Ft3 is not over range you are not over medicated

Need to test vitamin levels and TSH, Ft4 and Ft3 together

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water .

Last dose of Levothyroxine 24 hours prior to blood test. (taking delayed dose immediately after blood draw).

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Is this how you do your tests?

Private tests are available as NHS currently rarely tests Ft3 or thyroid antibodies or all relevant vitamins

List of private testing options

Medichecks Thyroid plus vitamins including folate (private blood draw required)

You need to test all four vitamins

If happy to do DiY fingerpick test, choose Thriva or Blue Horizon

If want to pay extra for private blood draw, Medichecks is another option

Obviously many clinics still not doing blood tests
