Help with results please

Hi, I take 100 mg of levothyroxine and 10 mcg of liothyronine and was just a little concerned that I’m taking too higher dose of liothyronine?

Tsh level 0.06 - range 0.25 - 5.00

Serum free T4 12.5 - range 9.00 - 23.00

Serum free T3 4 - range - 3.50 - 6.50

Any advice would be much appreciated.

Looking at previous posts ...you have Hashimoto’s?

For full Thyroid evaluation you need TSH, FT4 and FT3 tested. Also EXTREMELY important to regularly test vitamin D, folate, ferritin and B12

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and before eating or drinking anything other than water .

Last dose of Levothyroxine 24 hours prior to blood test. (taking delayed dose immediately after blood draw).

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

If/when also on T3 make sure to take last half or third of daily dose 8-12 hours prior to test, even if this means adjusting time or splitting of dose day before test

When were vitamin levels last tested

What vitamin supplements are you currently taking

Your Ft3 is nowhere near the top of the range so you are not overmedicated.

Tsh is not a reliable indicator of dose. You need to look at ft4 and Ft3.

Now if you are having symptoms it will be because either ft4 and Ft3 are not optimal for you.

Your TSH is suppressed because you take lio - it's just what happens. Your free T4 is 25% through range and free T3 is only 16.7%. Most of us need both to be much higher - at least in the top third of each range. So if you don't feel great, I'd suggest asking for another 25 mcg of levo and seeing what that does to improve things x

Thank you, I’m feeling a little achy and more tired recently so that’s really helpful advice. I shall speak to doctor about increasing my dose x

Your FT3 & FT4 are both well below mid-range. Have you ever been on a higher T4 dose and found you were T4-intolerant? If not, I would be inclined to say you should try increasing T4 dose to push FT4 to mid-range (about 16). If you tolerate that, see if that has increased FT3. If you get feelings of being too hyper while increasing T4, you could decrease your T3 dose by 1mcg for every 3mcg of your T4 increase, to see if that reduces the hyper feeling, until 6 weeks has passed so you can test again. If increasing T4 dose doesn't increase FT3, then go ahead with increasing T3 dose.

If you do have Hashi's, keep in mind that your body might respond in an "irrational" fashion to dose change.

Thank you for your really helpful advice. I never know what to try for the best to help me feel better. I definitely think liothyronine has helped but I’m still suffering symptoms unfortunately. I hope if I increase my T4 medication this may help then. I’m a runner and I’m really struggling with feeling so tired and achy! Thank you again 😊
