I’ve been on 137.5mcg Levothyroxine since December 2019 to date. I’m puzzled why the current results are less than optimal and fluctuating since December 2019 as I’ve been on the same dosage of 137.5mcg since December 2019.
Current symptoms, generally good, but lethargy sets in late afternoon; insomnia as always.

FT3 5.39 (3.5 - 6.5)
FT4 17.74 (11.5 - 22.7)
TSH 0.012 (0.27 - 4.20)
So that’s through the range:
FT3: 63.00 %
FT4: 55.71 %

FT3 5.16 (3.5 - 6.5)
FT4 19.97 (11.5 - 22.7)
TSH 0.014 (0.27 - 4.20)
So that’s through the range:
FT3: 55.33 %
FT4: 75.63 %

Results 23 December 2019
FT3 5.71 (3.5 - 6.5)
FT4 19.41 (11.5 - 22.7)
TSH 0.005 (0.27 - 4.20)
So that’s through the range:
FT3: 73.67 %
FT4: 70.63 %
All 9am blood draws, last Levothyroxine 24 hours prior, fasting, biotin discontinued 5 days prior.
Vit D, B12 optimised. Iron & Ferritin improving.

Levels do tend to jump around with Hashi's, as the thyroid is slowly destroyed, sometimes causing the dying cells to dump their store of hormone into the blood.
Then imperative for 6 weekly tests and ensuring a within range low TSH and FT3 FT4 in 60%-70% range ? Hence a dose increase warranted by say 12.5 mcg to 150 mcg to help me get back there ?
I don't think you should be aiming for any precise numbers. You want your levels to be wherever they make you feel well. That could be a lot higher than 60-70%. Blood tests are at best a vague guide. It's how you feel that is most important.
Understood, thank you. Comparatively I think I felt better back in December but it was all new unchartered territory, although I found it emotionally taxing at the time. But looking back over 2-3 months I feel the lethargy kicking back in slowly.
