Total T4/T3 tests aren't useful, they don't tell us anything much. It needs to be Free T4/T3. But taking NDT tends to lower TSH and Free T4 anyway, so it's the Free T3 result that's important.
FRee T3 2.3 2.3-4.2 pg/ml
Considering she's taking 1 grain NDT then her Free T3 is bottom of range. NDT contains T3 so her FT3 should be higher, this result suggests she needs to increase her dose of NDT. When optimally medicated one might expect FT3 to be in the upper part of the range, possibly the upper quarter if that's where she feels well.
How long has she been on 1 grain NDT? She could increase gradually, 1/4 of a grain at a time, possibly 1/2 a grain for the first increase considering how low her T3 is.
