But now feeling how i used to again with eyes pulling, yawning all day, insomnia and stabbing pains on the head.
what happened? i am still taking the same dose and not changed anything.

You are undermedicated to have a TSH over range, most Hypos feel best when it's downto around 1.
And you haven't got a FT3 result which is needed when taking T3.

You have Hashi's, as mentioned in this previous thread of yours, and with antibody activity results and symptoms will fluctuate healthunlocked.com/thyroidu...
Did you check out the links SlowDragon posted? Have you adopted a gluten free diet?
Have you got current test results for your vitamins and minerals? I know you said you are supplementing, but are you taking the right dose and are your levels optimal?
I say great that you are using T3 as I know how to use T3, but can't afford the cost of it in Canada. It cost me Cdn$350 for about 9 months to do my three trials of experimentation with it. If I could get it again, I know exactly how I would use it to drive down the reverse T3 which i successfully did in 2017, but the last test showed it had climbed again.
One woman I helped got her TSH from just over 5 to a lower healthy range just by taking 4 evenly spaced tablets with food of a professional grade mult that is available on amazon UK website and can be delivered to you. So I really think it is best to do all nutritionally also with added supplements rather than become diagnosed with a disease and then put on medications you say overdo things and make you more ill.
Well, you're obviously under-medicated. I'm wondering why you reduced your levo twice. Were you aiming at going T3 only? Were you a very, very bad converter - have you ever had your FT3 tested? Thing is, you reduced your levo from 75 mcg to 25 mcg - 50 mcg reduction - but only added in 3.25 mcg T3. How can that possibly make up the difference? Why did you reduce your levo at all if you were only going to add in such a tiny dose of T3? You're now on such a small dose over-all that I'm not surprised you're ill.
How long have you been taking selenium - just a shot in the dark here? Selenium doesn't suit everyone. On the other hand, you're possibly low on zinc. Low zinc causes muscle pains. Why not try stopping the selenium and adding in some zinc?
I very much doubt that Medichecks said you were 'going hyper' because it's a physical impossibility when you're hypo. They might have said you were over-medicated because your TSH was low, but I doubt your FT3 was over-range so you weren't over-medicated.
Yes, I've just looked at your Medicheck results, and your TSH is very low. But, that does not mean you are over-medicated. Your FT3 wasn't much over mid-range, so it was possible that you needed to add some T3. But the amount you added was rediculously low. One usually starts on 6.25 mcg. You are now under-medicated, according to your TSH. Why did you reduce the levo twice? Once was more than enough because you are not taking enough T3 to replace it. One normally starts by reducing the levo by 25 mcg and adding 6.25 mcg T3, which is the rough equivalent. The only thing you can possibly do is increase your T3.
The pain could have been due to many things. Not necessarily the T3. Start by increasing it to 6.25, and see how you go. You can increase by 6.25 every two weeks until you feel well.
Do you think there could maybe be an adrenal problem too? As I feel super exhausted by 3pm and feel like I've had an energy crash. This has been going on for months now.
There certainly could be an adrenal problem - but I'm not sure that would make you exhausted by 3 pm. You'd have to get your cortisol and DHEA tested to find out.
