A few weeks ago I posted up that I have had under active thyroid for 7 years and that I have not felt right and it got suggested that I have a full test of vitamins and antibodies to see if that is causing the problem.
Please find below my results and I would be grateful if anyone could give an opinion on them. The doctor from medichecks said my TSH if very low and my ferritin level is high which I knew but the doctor just said it is one of those thing.
TSH 0.28 range 0.27-4.20
FT4 free Thyroxine 19.900 12.00-22.00
FT3 4.89 3.10-6.80
Antibodies Thyrogulin 41.40 0.00 -115.00
Thyroid peroxidise antibodies 19.5 0.00-34.00
Vit D 68 50.00-200.00
vit B12 63.100 25.10-165.00
folate 4.88 2.91-50.00
ferritin 268 13.00-150.00

These are pretty good - TSH below 1, FT4 in the upper quarter of range, FT3 48% through it's range, a bit low but T4 to T3 conversion isn't bad at 4.06 : 1 (good conversion takes place when FT4:FT3 ratio is between 3:1 and 4:1).
To improve that conversion you could take selenium l-selenomethionine but no more 200mcg daily.
Antibodies Thyrogulin 41.40 0.00 -115.00
Thyroid peroxidise antibodies 19.5 0.00-34.00
I'd be slightly suspicious about these and want them retesting at some point. I'm not Hashi's and have had quite a few antibody tests over the years, mine have always come back less than 12 for both TPO and TG every time, they do fluctuate a bit but have never gone above 12. If those were my results, I'd be wondering if they ever go higher and suggest Hashi's.
Vit D 68 50.00-200.00
The Vit D Council recommends a level of 100-150nmol/L so you should be looking to supplement to improve your level. Taking 3000iu for 3 months might bring you into the recommended range, retest at that point and if you have reached the recommended level then reduce to a maintenance dose which may be 2000iu daily, maybe more or less, maybe less in summer than winter, it's trial and error so it's recommended to retest once or twice a year to keep within the recommended range. You can do this with a private fingerprick blood spot test with City Assays vitamindtest.org.uk/
There are important cofactors needed when taking D3 as recommended by the Vit D Council -
D3 aids absorption of calcium from food and K2-MK7 directs the calcium to bones and teeth where it is needed and away from arteries and soft tissues where it can be deposited and cause problems.
D3 and K2 are fat soluble so should be taken with the fattiest meal of the day, D3 four hours away from thyroid meds.
Magnesium helps D3 to work and comes in different forms, check to see which would suit you best and as it's calming it's best taken in the evening, four hours away from thyroid meds
Folate is far too low, it should be at least half way through it's range. A good B Complex such as Thorne Basic B or Igennus Super B will help raise your level, but check for B12 deficiency and do any testing for that before starting a B Complex as the folate (folic acid) will mask signs of B12 deficiency.
ferritin 268 13.00-150.00
I take it you don't supplement? Do you eat a lot of liver or iron rich foods? If this is a problem you could donate blood that would bring it down.
Have you had tests for hemochromatosis for high ferritin? It's probably nowhere near high enough, but could be checked out by GP
Hemochromatosis causes high iron levels and these can affect Thyroid
As SeasideSusie says, if that's the case blood donation can help
Personally, if you are feeling terrible, I would try strictly gluten free diet. Your antibodies are suspicious and many thyroid patients have leaky gut and gluten intolerance
Vitamin D is too low and low B vitamins are often linked to this as well
A good quality vitamin B complex should give benefit.
If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 3-5 days before any blood tests, as biotin can falsely affect test results
Vitamin D - Aiming to improve to around 100nmol. Vitamin D mouth spray by Better You is good as avoids poor gut function. Suggest you try 3000iu for 2-3 months and retest. It's trial and error what dose each person needs. Once you Improve level, very likely you will need on going maintenance dose to keep it there. Retesting twice yearly via vitamindtest.org.uk
