Your TPO antibodies are over range confirming you have autoimmune thyroid disease (Hashimoto's)
FT4 is on the low side.
Vitamins are all a bit low, but probably not low enough for GP to offer to treat
Ferritin should be half way in range, eating liver once a week should help improve levels
B12 , generally considered low if under 500, especially if folate is also low, which yours is
You could ask GP to test for Pernicious Anaemia but they are unlikely to think level is low enough. In which case self supplementing a good vitamin B complex with folate in, plus a daily sublingual B12 lozenge will help improve
No vitamin D test

Hashimoto's affects the gut and often leads to low stomach acid and then low vitamin levels
Low vitamin levels affect Thyroid hormone working
Poor gut function can lead leaky gut (literally holes in gut wall) this can cause food intolerances. Most common by far is gluten
According to Izabella Wentz the Thyroid Pharmacist approx 5% with Hashimoto's are coeliac, but over 80% find gluten free diet helps significantly. Either due to direct gluten intolerance (no test available) or due to leaky gut and gluten causing molecular mimicry (see Amy Myers link)
But don't be surprised that GP or endo never mention gut, gluten or low vitamins. Hashimoto's gut connection is very poorly understood. Many Medics deny low stomach acid even exists
Changing to a strictly gluten free diet may help reduce symptoms, help gut heal and slowly lower TPO antibodies
Ideally ask GP for coeliac blood test first
