Hypothyroidism and Omeprazole query

Hello knowledgeable peeps ... I have been diagnosed with LPR (laryngopharyngeal reflux or silent reflux) on the symptom of a cough I have had for 8 years ... the GP leapt to insist I take ghastly Omeprazole (20mg gastro resistant daily) - I had asked for referral to a chest consultant with a view to a bronchoscopy - which is the only examination I have not had over the past 8 years) but this was denied and the holy grail nasty little tablets came my way - I have hypothyroidism of 15 years standing and had an utterly horrid episode over the past 2 years but am now at long last stable again - as per most posts - I paid privately for bloods which showed I was lacking in several critical vits - I am on NDT (2 grains daily) and my TSH has come down from 44 to 0.75 and I almost feel well again - I did start to take the nasty PPI's but began to feel really ill so have now stopped and am doing an acid reflux diet (I have never had heartburn or any feeling of acid in my throat). I was wondering if anyone knows how PPI's like Omeprazole interact with NDT and all the expensive supplements for B12, Vit D, Selenium and others that I am taking? Thanks guys.

PPI's are renown for lowering vitamin levels. Especially B12 and magnesium, both essential for thyroid. Most hypos have low stomach acid not high

Thanks SlowDragon - yes I have made water Kefir, fermented foods, made wonderful bone broth and used liquorice and slippery elm to heal my gut which was so damaged over the past 12 - 18 months, I take inulin (pre-biotic for those reading who may not know what this is) and a good quality non-dairy acidophillus probiotic - I am dairy free so am at a loss about my cough - I just know in my heart that taking PPI's is not the right thing for me to do - will still stick with the diet but have a bottle of organic cider vinegar in my pantry! Think I need to push for an endoscopy to rule out LPR. Thyroid antibodies were low but still present so I do not have Hashimotos - I am fed up to the back teeth of coughing though!

Sounds like you are doing all the right things (and a lot more)

What are your thyroid results like?

Presumably you already tried upping your NDT?

JadisFox profile image
JadisFox in reply toSlowDragon

Yes I have upped my NDT - hence bloods have got a lot better and I actually feel kind of human and functioning again! I am going to do more full bloods in the next couple of weeks so will wait for those as I am 100% sure they will be better than the last lot! Thanks for the advice.

Ppis are handed out like smarties, i took them for 3months snd never again as they are poison to your body and brain, also rebound effect of coming off them is absolute dresdful. It turned out i was coughing due to thyroid nodules.x

JadisFox profile image
JadisFox in reply toRmichelle

I only took them for 3 days and came to the same conclusion about them as you did - was your throat swollen if you had thyroid nodules? I have considered this having been hypo for around 15 years now - what sort of cough did your thyroid nodules cause? I have a productive cough that is really quite bad and it wakes me at night - I often end up waking up due to choking, thanks for your input.

Rmichelle in reply toJadisFox

Originally hyper back in july but now hypo and hashis, when my cough first begin it was just like how you described and i could not lie down as it would always wake me and choke me, i am trying to fight a battle with endos at present as even before nodules were confirmed via thyroid scan i knew exactly what side they were on as i could feel them and i am sure they are causing the uncomfortable feeling on my trachea at times (like pressure but endo saying they are too small, i do alot of clearing my throat somedays and always get a build up of mucus aswell(sorry not pleasant)originally had swallowing problems with food and some days even drinking water it felt like i would choke, i know it is the thyroid causing this but at first i was given omprazole for acid reflux etc,i do have a hiatul hernia but useless go put 2 and 2 together and come up with 100!! I have been for endoscopies, barium swallow xrays, 2 larynoscopies, cat scan and not a dickie bird!! Mine started to improve on carbimazole but still do get it, omeprazole is poison it has nasty side effects and long term use is not good-it virtually blocks everything off, never again, i use gaviscon or zantac now for heartburn but it does not help the cough and i will do everything in my power to get nodules out!! Ooh gps put everything down to virus or acid reflux or anxiety now instead of investigating things. Hope this has been some help.😊x

This sounds like my history but have an Endo intent on Surgery. Just found RFA is being done on NHS at UCLH so ditched the Endo and my GP is referring me to UCHL. Fingers crossed

JadisFox in reply toRachelvc

That's interesting to know - and as we are all supposed to have access to treatment anywhere we chose (as in chose and book) I shall bear this in mind! Thank you!

Rachelvc in reply toRachelvc

Thank you for this info. I too have now been referred to UCLH. Hoping they will be able to sort it out. One of my nodules is a Complex 3. I will report back.

Yes thank you - very helpful - I did suggest a thyroid scan for nodules at my most recent visit but my GP just looked really irritated and said a flat no - just like he did with my suggestion of an endoscopy (to rule out silent reflux at least) he stated quite firmly that he would not refer me - it was quite a shock - I am not used to not getting on with people but I am quite convinced now that there is something on my medical notes - or at the very least a discussion has taken place about me all because I had the temerity to refuse synthetic thyroid drugs and decided to buy my own from abroad! My problem is I have never had indigestion or any other symptom of reflux but I have had a feeling of something in my throat (bolus sensation as the ENT consultant called it) - I do choke on food as sometimes it feels like it will simply not go down and I too now have a small sliding hiatus hernia - which I firmly believe has been caused by over 8 years of strenuous coughing! So did your laryngoscopy show damage from acid reflux? My voice box (as far as an ENT consultant can go) showed no evidence of acid reflux. I need to get as much evidence as I can to take back with me for round 2!!

Rmichelle profile image
Rmichelle in reply toJadisFox

Sorry just read message, ent did say some reflux damage andvi did take omprazole but did not want to take something as strong as them, if it was life and death i probably would but i would try other alternatives 1st.

I am quite fortunate really as therecis a couple of gps in my surgery that will refer you for whatever you ask for, 1st endo i saw said no to a thyroid scan as he said you dont have nodules, i cant feel them, so off i went to my gp and said about all my problems and he felt my neck aftervi told him where i could actually feel them in my throat-and guess what he could feel them aswell, so he sent for a thyroid scan, i took greatbpleasure in telling that endo i would not be seeing him again. if you arevhaving swallowing problemsvit should be looked into and not chucked ppis because they are cheaper than a endoscopy, i think you should push for a thyroid scan and then endoscopy, i would go for it. Good luck let us know what happens? 😊

I think ent meant globus sensation(in other words anxiety, lump feelingin throat)it is not!!

JadisFox profile image
JadisFox in reply toRmichelle

Yes that is what I meant to say - so yet another specialist who thinks I am a nut job!

JadisFox profile image
JadisFox in reply toRmichelle

Will do Rmichelle - and thank you - true what you say about pills being cheaper than proper investigations but in the long term? I am with you on the alternatives and will try finding a sympathetic GP! I shall let you know how I get on!

Rmichelle in reply toJadisFox

Yes please do let me know., yiu are not a nut job these are real things-i believe you.xx

JadisFox in reply toRmichelle

Thank you for that - I do wonder at times about my own sanity - its a bit like that film "Vanilla Sky" - you begin to question reality in the end!

Rmichelle in reply toJadisFox

Yes i understand, before my diagnosis of thyroid disease gps gave me the run around and said "virus on 2 occassions, anxiety (never suffered with that), asthma, blood clot on my lung in which they thinned my blood for!!and had cat scan-all clear, barium swallows , you know the story!! So i git fed up with gps saying nothing wrong and done my own research and thought thyroid and iron levels and wham bam!! Postive result. I have no confidence in the medical profession, it is a long journey on a rocky road this thyroid business and im sure you have been told that the thyroid can mess with other parts of the body and mind, there are los of people on this site with the same problem. We know our bodies better than any gp- my sister was very poorly for years with gynae problems and knew there was somethi g wrong with her womb-it took her 4 years of fighting consultants and gps who told her she was imagining things-after all them years they found she had a tumour growing inside her womb-thankgod it was benign!! This is what she told them in the beggining She had a letter of apology from the bma a d gps surgery and she reported it to the national newspapers who printed her story.just goes to show we know better not some idiot gp wbo does not think out of the box. Keep fighting!!.xx

I popped - Coughing and Low B12 - into the Search Thyroid UK box and there are 175 previous discussions on the topic :-) The above link takes you to a research paper about it. What was your level before you started supplementing B12 ?

JadisFox profile image
JadisFox

Wow! Seems I need to use my noddle a bit more now that my head is fog free! I should have of course tried that and will follow your link to have a good old read of it - my B12 was a bit low but not critically low (like my Vit D was!) so as I know your body cannot store B12 I decided to supplement sublingually - even after I had shown my GP my blood test results - the GP was of course, non-plussed and did not think I needed to supplement - but as we are all finding out ..... what do they know eh? I am having my bloods done again in a couple of weeks so will wait until then to no doubt see an improvement. Thank you for your help.

When supplementing B12 also take a B Complex to keep all the B's in balance. Which B12 are you taking and how much ? What was your result before supplementing ?

JadisFox profile image
JadisFox

I did take a liquid B complex with B12 a while ago but developed a very angry and itchy rash on the back of my hands, on my neck and part of my face - I stopped the B complex but have carried on with the B12 and the rash has almost gone - have you heard of that before? I will fish out the file with my blood results in but my B12 was not terribly low - I am just getting ready for work ATM (yes I finally felt well enough to return to work!) so will have to get back to you later on this evening. Thanks

I work for an Osteopath and order my supplements through work - they are expensive but are very good.

I saw your post from months ago regarding LPR (silent reflux).

The best test for throat reflux is the "24-Hour Multi-Channel pH Impedance Test." The standard single pH probe only checks acid in esophagus. Patients with throat reflux issues need dual probe in both larynx and esophagus which tells what is being refluxed and when. It can be acid, or pepsin/bile, so that would be the best test. Also, an endoscopy often shows no damage for patients with LPR.

I'm suffering from LPR now, and it's been pretty tough. Good luck --
