The short answer is yes - and now. All those levels are pitifully low and need to be raised post haste by infusions/injections where appropriate.

You need to see your GP in the morning and start treatment. Your GP should arrange immediate B12 injection and other tests you need. You may need iron infusions.
You should be given a prescription for high dose of Vitamin D. (Loading dose of 280,000 to 300,000 IU to be taken in split doses over several weeks) before continuing on daily maintenance dose of between 800 and 2000 IU. You should also receive a prescription for Folic Acid 5mg tablets , but must not take them until after you have started B12 injections. You need immediate B12 injection. Iron investigated and immediate treatment. Loading dose Vitamin D & Folic Acid tablets after you have started B12 injections.
When were these tests taken?
Who will be treating you ? GP or Haematologist?

You need a little supplements - like a bucket full! Looking at your other post you are undermedicated. Low thyroid hormone causes these deficiencies.
Please ring your doctor's surgery tomorrow morning and ask for an urgent same day appointment. These are some of the lowest vitamin levels I've seen on this forum and you need immediate treatment. Do not delay. Do not allow the receptionist to put you off - tell them you have critical results that need immediate attentio or you will be forced to attend A&E if suddenly unwell and the practice will be billed for it if nothing has been done following results (I think).
If you're feeling very unwell it's hard to be assertive so if you don't get any joy, get someone to help you make the urgent appointment.

Sasha, Looking at your other post you were prescribed supplements and they were stopped. In particular your iron is exceptionally low. With you still being somewhat hypothryoid and having some very low mineral / vitamin levels I can imagine you will have quite substantial symptoms and it is very difficult to be assertive, in control and to remember things. When you have a doctors appointment take someone along with you if you can and ask how they are going to investigate and treat these deficiencies. The low ferritin is a major concern and absolutely should be addressed as a matter of urgency. (Just double check you have given us the correct number for the ferritin as I've never seen one so low).
You need an iron infusion, a loading dose of vitamin D according to the guidelines for your area, and to be tested for pernicious anaemia. If your GP has done nothing about these results s/he needs reporting for negligence.
