Obviously these are dire and the cause of low TSH and high FT4
Low vitamins stop thyroid hormones working
How often are you getting B12 injections? Not frequently enough looking at this result
See this post for similar levels and how to improve

Thankyou I am getting B12 injections once every 3 months and I have been prescribed iron to take once a day and folic acid once a day

I would post these results on

They are B12 experts
Did you have frequent doses when first started on B12?
Ferritin is dire. You probably need iron infusion
Plus dose increased perhaps to three a day, take with vitamin C to aid absorption. Eating liver once a week will help improve too
Folate. Try adding good vitamin B complex too that contains folate not folic acid
Vitamin D - your GP should be prescribing LOADING dose
Make urgent appointment with different GP if first one is unhelpful

How long have you been getting B12 injections? As SlowDragon suggests also put your information on the Pernicious Anaemia forum. Your B12 is far too low unless you've just been diagnosed.
