Help with my Mum's results, please.

My Mum has had a horrible couple of months. Here are the last results from 11/15 to 11/20. She was on 125mcg of Levothyrox

TSH 9.45 uIU/mL Range 0.27-4.20

Free T4 1.4 Range 0.9-1.7 ng/dL

PTH Intact Result 100 pg/ml Range 15-65

She has had terrible constipation, body aches, sudden hand and wrist pain, back pain, and slept horribly etc. I know her TSH was quite high which would explain why she was so miserable. However, I want to reach out for some feedback as to how to proceed?

My Mum was on 125mcg of T4 Levothyrox? so it appears she was under medicated. She was previously at 135mcg and 150mcg, but the Primary Care Physician kept changing the thyroid medicine as she was either over or under medicated with the Hashimotos. I suspect maybe my Mum is all of a sudden not assimilating her T4 medicine well? I see a similar pattern as with myself, I had to take alot of T4 years ago to get a smudge of T3.

I managed to get my Mum at least under Tirosint to see if would help her a little.

I think she might felt a tiny bit better, but we have a long ways to go.

I would love to put my Mum on a combination of T4 and T4, or pig thyroid or even T3 only. However, she does have hypertension, atrial fibrillation and she is 75 years old. She also has had really odd nosebleeds out of nowhere so I thought they could have been from the hypertension. Of course, her blood pressure has been ridiculously high, I believe because her TSH is so far off from being optimal. Finally, my Mum has no stamina, no appetite and has anorexia thoughts. She is repulsed by food and it is a struggle to get her to eat. I doubt she even gets in 600 calories a day, again no appetite. I researched a correlation with low t3 and anorexia.

This thyroid panel is from her Primary Care Physician, but she couldn't figure my Mum out with her dosage so she was sent to a Endocrinologist. My Mum became so desperate so she showed up one day and begged to see the Endo as she had to wait several months just for an appointment. The Endo. took a blood test ONLY to come up with a TSH and a Parathyroid number. I will post these results once I receive them via mail.

However, I do remember confirming over the phone the TSH did come down to 6.04 approximately, but the Parathyroid test well over 100. The Endo has now increased her to 135mcg.

I have been authorized by my Mum to talk to her Endo's nurse. I told her all these findings and requested my Mum be seen as reasonably possible especially since I think the lack of T3 is causing her blood pressure to spike...plus all the issues.

It has been 2 days, the Endo's nurse has not returned my call.

Please share your thoughts..I want to drastically help my Mum, but my concern is how could we self-medicate with the issues mentioned???

I am sorry to hear about your poor Mum. She is elderly & needs to be treated with much care but I had hypertension & atrial fibrillation when medicated T4 only. Her TSH level is also much too high.

Introducing T3 helped elevate so many ailments but before doing this, I would have your Mum's T3 levels tested. Also Vit D, Vit B12, folate, ferratin, and calcium given she is having PTH investigation.

A g/f diet will reduce inflammation and help any thyroid meds work better.

Radd and Clutter- Thank you so much for your reply. I totally agree I think getting her Free T3 will do wonders. However, how could it be done safely with her issues if the Endo will not help? I imagine it will be the same as always...a flat out No! to go a combination T4 or T3 thyroid medication or T3 only?

All the issues in my mind are due to no T3 in her body...at least this is what I went through.

I am not sure I understand the PTH Intact? I do show her Calcium to be a 9.0 mg/dL on a Range 8.4-10.2 so it appears OK?

And Corrected Calcium 9.6 mg/dL Range 8.4-10.2?

She did see her Cardiologist yesterday to ask about the high blood presure so a new medicine was prescribed to better control it. However, the Cardiologist affirmed all the aches, lack in stamina, appetite, problems with sleep etc. are not related to her heart.

I think my Mum had been taking a Vit D supplement of 50,000 every Sun or every Sunday? My concern is no they was never any Vitamin K added?

I pushed the Endo's office to do a complete thyroid panel the next time to especially test the Free T3 and Free T4, plus I will ask to check Vit D, folate, and ferritin. However, I think I checked these a few months back from her old Dr. and the levels weren't bad.

If I get nowhere with the Endo. How could I potentially treat my Mum? She is so uncomfortable...🤧

Your mother's calcium and phosphate should have been tested at the same time as PTH to determine whether she has hyperparathyroidism. labtestsonline.org.uk/under...

Your mother was also undermedicated. Your mother should have a follow up thyroid test 6-8 weeks after increasing dose to 135mcg to ensure dose is adequate.

You need to get some advice on high cal foods to make sure your mother is getting enough nutrients until her appetite improves.
