HbA1c and glucose are considerably down. Are you still taking metformin?
Low creatinine:
Low blood levels of creatinine are not common, but they are also not usually a cause for concern. They can be seen with conditions that result in decreased muscle mass. Levels of 24-hour urine creatinine are evaluated with blood levels as part of a creatinine clearance test.2 Feb 2017

Other results are within range (normal) except cholesterol which doesn't have a range by it. Cholesterol may be higher than normal because TSH is high and will probably drop once you are optimally dosed.
If B12 and folate are dropping you might want to supplement. 1,000mcg methylcobalamin every 2-3 days will probably be enough B12 and a B Complex vitamin will give a little boost to folate.
