How long have you taken supplements for? B12 is very low and taking folate supplements can mask a B12 deficiency. Has your GP ruled out pernicious anaemia before starting you on folate?
ferrous fumarate since 2014 and folic acid since 2015 vit d since 2013 haven't had pernicious anaemia ruled out before starting the folate thanks
Post your B12 and folate results along with information about your other deficiencies along with supplements on HealthUnlocked Pernicious Anaemia forum to get best advice on how your GP should treat B vitamins deficiencies.
I'm pretty sure you should have had tests for intrinsic factor antibodies to rule out pernicious anaemia before supplementing with folate. B12 deficiency has long term consequences so you really need to check it out.

The fact that all other results are under range despite the level of supplements indicates a possible underlying absorption problem. So rule out pernicious anaemia and you may need to rule out coeliac disease too.
What has your GP said about these results?
Please get tested for h pylori. It effects your b12 and iron. A b12 def will present with higher mcv, macrocytosis. an iron def will present with lower mcv, microcytosis. When you have both your mcv can be in the normal range.
Taking folate can mask a high mcv.
H pylori effects both b12 and iron absorbtion and you will get worse slowly so the symptoms are hard to pin point along the way but eventually you feel like you cant exercise, get breathless from minor activities, walking up a flight of stairs causes your thighs to feel heavy/tired/burn. Memory and concentration issues, possible dizziness, cold hands/feet/nose. Lower body temperature. Possible Stomach pain or maybe just nausea or lowered appetite. Joints hurt, muscle weakness. The list goes on.
H pylori is a simple test. And your thyroid will not work properly without these nutrients.
