Blood advice please.

Hi all, 150 levo blood TSH 0.3. FT4. 20.5, FT3. 4.4. Not feeling great so I increased my own dose to 175 levo for 7 weeks, Blue horizon test as follows:

CRP 1.3 < 3.0, Ferritin. 135.9. (30-400). Serum folate. 32.4. (10-42). B12. 472

TPO. 582. <34. TGB. 510. <115

TSH 0.03. (0.2-4.2). FT4. 23.2. (12-22). FT3. 5.1. (3.1-6.8). Total T4. 153. (64-142).

Feel better at this but still have some symptoms : heady most days ,dry skin,hair loss,just run down.

Would I be right in thinking that my TT4 would indicate a need for a little T3 to balance out the numbers?

Any advice appreciated.

Clubby, TT4 is T4 bound to proteins, and isn't useful thyroid measurement when Free T4 and Free T3 which measures the unbound proteins, ie the T4 and T3 available for use, are available.

TSH 0.03 is suppressed (very low). This is to be expected when patients are taking >145mcg Levothyroxine. FT4 23.22 is mildly over range but okay because your FT3 5.1, which is good, is within range, ie you aren't overmedicated.

Antibodies are positive for autoimmune thyroid disease (Hashimoto's). You may benefit from improvement in symptoms and reduction in Hashi flares and antibodies if you adopt 100% gluten-free diet.

B12 472 is unlikely to be deficient, but 1,000 is optimal. You could supplement 500-1,000mcg methylcobalamin plus a B Complex vitamin. CRP, ferritin and folate look good.

Hi thanks for my only reply, would a trade of levo for some t3 be of any help to balance the numbers a little and help with symptoms?

Clubb29, I don't think your numbers are unbalanced, but you might feel better with a little less T4 and some T3. FT3 5.1 is good so you may not feel the improvement someone with low FT3 will experience. It may be worth trying a 25mcg reduction in T4 with 6.25mcg T3.

Hi thanks again for advice,going to endo on Tuesday the more info I can take the better. I haven't done the gluten free approach yet I guess I was hoping I wouldn't have to. Not expecting much from endo and I'm sick of being sick so I guess it's time for some GF reading.
