Back in March I was diagnosed with an underactive thyroid along with Pernicious anaemia and Vitamin D deficiency.

I started on levothyroxine 25mg not knowing my actual results. I checked with my nurse today who gave me my last lit of results.

T4 = 11.8 (11-25)

TSH = 9.2 (0.27-4.20)

Can anyone help me understand what this actually means?

Welcome to the forum, Vampnation.

Thyroid stimulating hormone (TSH) rises in response to low thyroid hormone (T4 and T3) and drops when sufficient hormone is detected.

T4 is a storage hormone which is converted in the thyroid, liver and other organs to T3 which is the active thyroid hormone.

T4 11.8 is very low in range and inadequate to convert sufficient T3. TSH has risen to >9 to nudge the thyroid gland to produce more hormone.

Dose increases are usually in 25mcg increments until TSH is just above or below 1.0 with T4 in the upper range. You should have a thyroid blood test 6-8 weeks after every dose adjustment. It's best to have early morning and fasting blood draws when TSH is highest and Levothyroxine should always be taken after a blood draw.

Thank you for your reply. I'm getting a better understanding now. In your opinion do my results indicate that I have a very underactive thyroid?

Vampnation, yes, they do. Are those results from March, before you started 25mcg, or more recently since you've been taking it?

The results are from May. I don't have my initial results from Match. I've now been prescribed 50mg with a view to a review and blood tests in 2 months time. I've been having regular B12 injections but still not feeling quite right so I questioned tge nurse today who confirmed my dosage too low but no other info.

Vampnation, Ask for printouts or make a note of results and ranges after every blood test. Your dose should have been raised in Apr/May. You won't feel right until TSH is around 1.0 but you'll feel better as it reduces.

Thank you for all your help! I was initially diagnosed whilst admitted into hospital due to critically low B12 results. Never had a follow up appointment with the consultant as he left to work in a different hospital! I will most definitely ask for all print outs in future! Once again, thank you :-)

On the B12 front, I suggest you hop over to the Pernicious Anaemia Society forum right here on HealthUnlocked. All too many people are poorly treated for that as well.
