Approval of Imbruvica Frontline for 17p deleted CLL and Important Trials
Hi Friends, The recent approval of ibrutinib for 17p deletion frontline hopefully is the opening wedge of more indications for treatment naive patients for it and other non-chemo choices. I have posted why this is so important and a list with links to some important clinical trials on my latest blog

Zydelig: The Black Box Warnings for use in CLL
Hi Friends, On my blog (http://bkoffman.blogspot.com), I have commented on the extensive black box warnings on the label for idelalisib or ZYDELIG that is a stark contrast to no black box for ibrutinib, not that
IMBRUVICA
doesn't come with its own set of cautions and concerns.

Imbruvica/Ibrutinib joint pain side effects
Because joint pain has been experienced by patients on Imbru/Ibru and others have not had this side effect, it is wise to explore all possible causes. I just found this article on a new virus to the US. It has a cool name "Chikungunya" and you thought new drug names were hard to pronounce;-) This virus

Update on PCYC-1115-CA Randomised, Phase 3 study of Ibrutinib/Imbruvica versus Chlorambucil in Patients 65 yrs + and untreated with CLL/SLL.
Day 1 of cycle 12 today and with the exception of my platelets, (fluctuating around 100) and my low Immunoglobulins, all counts are in the normal range. Lymphocytes 2.4, LDH 222, B2m 2.8 (a month ago) and WBC 7.5. Biochemistry all in normal range also. A CT scan and possibly BMB next month will reveal

Less than the prescribed dosage of Ibrutinib ( Imbruvica) ?
One of the prescribed dosages of
Imbruvica
for one patient with CLL is 140mg, 3 capsules, once per day. Has anyone with CLL experienced taking less than that amount and if so, What have been the results, if any ?

3 years on Ibrutinib/Imbruvica and a study involving fasting
If any members are contemplating getting Ibrutinib/
Imbruvica
and are worried by recent postings of a possible connection to cataracts I would have to say that it would be a very rare condition and not directly related to Ibru/Imbru.

CT scan series part 2 - a little about me
I have, however, made an effort to study CLL/SLL and have been around the block with failed standard therapies which damaged my kidneys and led me to participate in a Phase I-b Clinical Trial for a drug called PCI-32765 renamed Ibrutinib or
Imbruvica
.

Beta 2 Microglobin - an exquisitely sensitive marker for disease activity in many hematologic disorders.
to 3.3 (as of 27 March) since taking
Imbruvica
:) According to Michael Keating, 'The beta-2-microglobulin level has been confirmed as a major prognostic factor'. In my experience, the B2M is not routinely given. Prior to the study I have always had to ask for it.

Ibrutinib dosing rationale for CLL at 420mg per day
Even if it is called
Imbruvica
- same drug in the same 140mg dose capsule! Challenged by lbs to kg conversion?? calculate your minimum Ibrutinib dose and see if you need more than 420mg per day.
