What else can I do?

It started with hypersensitivity in my glans. Then I had a poking pain at the tip of the penis. It's been 1.5 years.

I noticed that lying on my right side or occasionally lying flat in my back will produce the poking sensation in the tip if the penis. I have avoided sitting down too.

I have had 2 MRIs, pelvic floor and lower spine. And they don't show anything of concern.

I went to my urologist and took 2 months of antibiotics but didn't help.

I went to a pelvic floor PT for 6 months, she did internal massage and gave me stretches to do. It didn't help much and I would get random flare ups after doing the stretches.

I just spoke with a doctor and he said I should try to do an ganglion of impar block to see if the pain goes away.

Any suggestions on what I can do next?

It sounds like nerve pain. I had similar pain and nerve pain meds helped hugely. It also stops the brain /pain cycle . I went to pfpt for over a year too

I am on gabapentin for a while now but didn't see too much improvement.

No not gab...

Probably amitriptyline or Cymbalta

painkillers and anti-inflammatories for now it's you which can reduce my pain and symptoms.

ibubrophene 400 mg at the time when I suffer too much
