Pain management for anal fissure induced anal spasm?

I have an anal fissure and am awaiting a colonoscophy under anaesthetic. After bowel movements, whether my stools are soft or even runny, my fissure cracks open and I often go into an anal spasm. These can change in intensity and duration - but not unusual to have a 12hr spasm. I feel as if I am losing my mind. I am utterly exhausted, and obviously am doing all the things: eating vegetables, taking stool softeners, using calcium channel blockers ointment, keep the anus clean, not forcing stools, sitz baths in Epsom salts, eating magensium rich foods etc etc. I am specifically asking this forum about PAIN MANAGEMENT. So far, I have only been told to take Asprin and Paracetamol. P

Start going to a physiotherapist to learn how to relax your sphincter muscles. Breathing in through the diaphragm and our through the bra strap area. Do not have surgery or your problems will be worse. Trust me on this!

Your muscles are probably too tight.

Been suffering like you for 3 yrs. My gastro doctor told me it takes a long time to heal and to use Recticare ointment which is 5% lidocaine. Can be purchased at drug stores located with hemorrhoid cream. Apply after each BM. It may sound strange but I also massage A&D ointment down there prior to having a BM and throughout the day, it keeps the area more moist and not so painful when the Anus needs to stretch to use the bathroom because that is the cycle, it’s like having a crack at the corner of you mouth, every time you open your mouth the crack opens back up and hurts. I also use Natural Calm daily which is Magnesium powder that you mix with water or juice. Keeps your stool softer to pass and has the benefit of helping you to feel less on edge. I Was on Valium suppositories for 9 months but I couldn’t really feel that it helped the spasms. I weaned myself off because it gave me a bad case of brain fog. Stole my memories of everything but am glad to say after about 4 months memories have returned. I also had a colonoscopy whic did not show anything but It put me into a severe pain flair for weeks, hopefully for you this will not happen. Gel Ice packs have been my saving grace when I have to sit down. For me any kind of heat to the area causes pain. Hope this helps a little and you feel better soon.

Following up on this post 9 months later. I am dealing with this for just 1 month and I can't imagine dealing with it for a year. Did you find any relief?

Hello, my husband currently has at least one fissure. He was on Nefedipine .3%, Nitroglygerin .2%. He's now on diltizem 2%, Bethanechol .1% and ISDN .2%. Do you have fissure(s)? It's been since December 2019 that he's had the fissure. We are pretty sure he has some form of PN, is seeing a pelvic floor therapist virtually via video call due to he is not able to ride in the car. He's considering botox to the spchinter muscle but the doc is 1 hour away. He's also considering nerve block but still 1 hour away. Hope you are well, God Bless❤
