Anal Fissure

Hello everyone. In May 2018 i started having unbearable pain during and after bowel movements. I tried to tough it up and only went to see a doctor after i noticed i was bleeding during bowel movements. The doctor diagnosed this as an anal fissure and i was perscribed medication and suppositries to soften stools. I was instructed to take said medication for 2 weeks. Luckily the fissure healed and things were back to normal - or so i thought.

Fast forward to september and the pain was back. Only this time it was ten times worse. The previous treatment i took did not. Work this time. So my doctor prescribed muscle relaxants and another medication. This treatment did not work and the condition got worse as the muscle spasms were cauisng a horrendous pain. So the doctor prescribed muscle relaxants and rectogesic. I basically spend the entire day literally in pain. Using the bathroom is utter agony and the rectogesic gives me a debilitating headache for which i take pain killers but the throbbing persists. I am currently taking diltizam 2% and the pain is horrendous. Luckily i am rid of the headache from the rectogesic. Does anyone have any feedback or information that would help. I would be very thankful

Eat dates and prune juice daily and bran flakes/brown bread. Also buy a sitz bath and soak a few times a day. Good luck.

Thank you for the tips! I have been relying heavily on dates and drinking lots of water. I hope it eventually heals

I can thoroughly recommend Botox treatment. I've had a fissure twice after having a failed sphicterotomy which left me leaking. Wish I had had the Botox first. I use Coloplast Comfeel barrier cream after every bowel movement and use wetted toilet paper to wipe as dry toilet paper is too harsh when used. If you can use a toilet near to a tap take single sheet and wet only half of it. Fold the dry half on top of the wet half and wipe quickly to avoid the toilet paper disintegrating. Try anusol suppositories too. Also, keep your stools soft by drinking lots of water and eat fresh fruit and veg, avoiding spicy food and limit the amount of meat and other constipating foods (Google that) and stay on the toilet long enough to make sure your bowels are fully empty to avoid going too often as far as possible. I get the barrier cream (4 tubes at a time) and anusol (90 suppositories at a time) on prescription in the UK. DO NOT have a sphicterotomy until you've had Botox and only as a last resort and my consultant has told me never to have anal flap surgery. Good luck and get well soon.

Thank you so much for your valuble reply. You are absolutely right, i saw a colorectal surgeon and she specifically advised me against having a sphicterotomy she said as my muscles are already weak from my pregnancies and deliveries. She suggested i try the diatezem 2% for 6 weeks and if it does not heal then go in for botox treatment which would be 70-80% successful and would have to be done under general anesthetic. Unfortunately i am unable to take suppositries because of the muscle spasm so like you said i try to drink loads and loads of water and i have been eating dates for the fibre and using the bidet. Also i did in fact notice that the pain is less when i avoid eating meats so i will probably become a vegetarian now. Thank you so much for your well wishes and positive feedback. It is greatly appreciated!

I am in the same position! I am going out of my mind with pain and am doing all the water/fibre/stool softener things and the only relief I get is if I stop eating so that nothing passes through my bowels. Even runny and soft stools are causing me to spasm severely.

I am so sorry to hear that you too are going through this dibelitating condition. As horrendous as it is you still need to have bowel movements otherwise you risk becoming constipated which will only make matters worse. I have been applying sudocream recently and it seems to be making things a bit better. If you need to vent or explain to someone who literally knows what you’re going through feel free to contact me

Yes, I know that I do. I tried fasting and then got impacted stool which was horrendous to dislodge.

I have been wondering about sudocream. Are you applying as a daily thing? Before you pass stool?

Yes i am applying it once in the morning after a bowel movement. Also another thing is that i am using a small stool to put my feet on during the bowel movement and to help with the spasms i read a tip from someone that if you blow out of your mouth while you have the bowel movement it will help ease the muscle spasm and it actually works. Just keep hydrated and dont overdo the fibre in your diet. I think its different from one person to another. Find the foods that ease your bowel movements. I depend heavily on dates. Try to see a doctor as well. I tried rectogesic for a while but the headaches left me unable to function but some people tolerate it more than others

youtu.be/b0uJlirFW3EThis can help you. You can control the paint by changing your diet. Also Please take high fiber diet like beans, nuts veggies, and berries.

Thank you for your advice. The youtube video doesnt work it says an error occurred. I would advise against nuts though because they would be painful

Careful with going straight to a vegetarian diet to avoid protein. I just had surgery 14 days ago and it’s been a nightmare. I decided to stop eating meat for softer stools and the first morning my bowl movement was much better but I made a massive mistake that night and added beans to my dinner. Oh my goodness it was a waterfall for literally 4 hours. U think a solid stool is painful. Wait until that stomach acid hits that fissure for 4 hours. I was in fetal position for half a day. I am really struggling. I am wondering if I did the right thing but I had three fissures and was in so much pain I did not really have an option. I would love to hear from anyone that made it through this. I feel defeated. I have two jobs, work from home, take care of my three kids full time and I am struggling.
