In 2014 the itch went ballistic for me, it took until 2016 to get it under control and I am now prescribed colestyramine 2 sachets one either side of breakfast, when that did not work well Naltrexone was added but I had a bad reaction to it so Rifampicin was prescribed instead and later 25mg sertraline. Although this is quite a lot of medication I can live a more comfortable life. When the itch still breaks through I use a 2% menthol cream straight from the fridge. I know some people use ice to cool the skin and others find that anti histamines work but you say you have tried all sorts which I assume are over the counter medications. It may be you need to speak to the doctor. There is a good article by Prof. Neuburger on the itch in the Bear Facts magazine of spring 2015. Although written fours years ago I do believe it is still current. Alternatively are there any research studies in your area that are tackling the itch - maybe worth an ask of your local consultant or if you are already taking medication I wonder if you could be referred to a centre of excellence such as Newcastle if you live in the area. I think London and Cambridge are also centres of PBC excellence but that would need to be checked.

The best thing for the itching is to not start scratching. It just makes it worse. I know how hard that is but it is true. There is nothing topical that works.
Ask your doctor about Fenafibrate……it is used for cholesterol but controls itching.
