I don’t have much to add as I also suffer with severe back shoulder, hip pain especially at night. 1 thing other than the meds that I’ve found helps a little is a hot bath of an evening. Try a heat patch in the area to fill the ache. I also have a shiatsu body massager which at times can help but other times can be agony.
Have you tried taking a small ball, a small pink ball is perfect or if you can’t get one a tennis ball and at shoulder height arm outstretched locking the elbow put the ball against a wall in front of your body.
Now move the ball up and down for a minute ( use a timer start 30 seconds and move the time up til you can do the minute) Repeat 3 times resting in between pressing hard and going up and down as fast as possible. When done 3 sets,
Then repeat the exercise going side to side.
Make sure to take breaks in between

Another exercise is lifting 5 lbs to start and moving up. 3 sets of 10. Lifting the weights on a diagonal from waist to shoulder keep locked elbows. Keep your thumbs up!
Both exercises target strengthening those hard to reach muscles behind the scapula doing lightly then once stronger you may not hurt so much.
I had similar sort of pain and they found a collapsed vertebrae in the region of where your bra strap runs across your back due to osteoporosis when I was 48. Not sure if that helps but same areas of pain plus some radiating around my ribs.
