Do you hate when you go to the doctor and they ask what your medicines are and if you've had any changes lately. Your mind then goes back over the long list of medicines and supplements you are currently taking and you're supposed to know milligrams and of dosage etc . It's darn near impossible to do it right. But I solved this dilemma. I made a list in my computer and titled it, "My Medicine" and I put each medicine I'm on, on the list. I try to keep the size of a credit card. Besides the medicine I put the dosage I'm on and then how many times a day I take it. I repeat the process until I put everything on it. By the time I finish. I've got 10 or 12 items listed. The medicines go 1st then I put the supplements on the bottom. Whenever you go to the doctor now, all you have to do is pull out the card and they are instantly up-to-date with our medicines regimine. It is also a great tool if you need any kind of help like being in a wreck, or any time you have to go to the ER so they have a list of your amedicines that is complete and easy to follow. On the back of this handy dandy little card I list my doctors complete name, speciality, address and phone
number. I usually can fit 3 or 4 of my primary doctors on it. My p.c., neurologist, and anyone else that is significant and t you see semi regularly. The really good part of this is you keep your medicines up-to-date by simply getting on your computer and updating your medicine list whenever medicines change. After I make a list of I usually print it out on cardstock or something stiff that will hold up a little better. Then I use a self laminating business card two sided pouch to put the card in it.
Mine looks something like this:
J. Doe Medicine List
Ocrevus XX mg every 6 months.
Neuroton XX milligrams X time day.
Bacolfen XXmg Xday
Etc etc....
Supplement list
Vitamin D3 X milligrams day
Probiotic
Cranberry supplement
Etc etc....
The doctors all love it and have said over and over again they wish more people would you do this because it makes their work so much easier. If you have questions or uncertain

Excellent suggestion, Fancy59 ! I have an Excel spreadsheet with my medication and supplements all listed and I print a copy and bring that to my doctors appointment. I like the idea of listing your doctors as well, just in case. Keep Smiling
Yours is better, Fancy59 , because you don’t need to rely on a computer. I just used that because I could store all my information in one place. Plus it kept me engaged to keep it current.
I love this idea! I line my meds up like a perp line up and take a picture on my iPhone but now it’s three pictures worth and annoying to list. Thank you for sharing!
I keep a list of meds in my purse and print a new one up before a dr appointment. Adding doctor's info is a good idea. I went to the walk in for a sinus infection last Sunday and I brought my list and the nurse thanked me so much for doing that. She said it is so hard to get people to put it all on a list. So much easier than writing it all out on the small space on the forms you have to fill out.

I do love it when my patients pull out a paper with their medicine list! Also, if you have multiple allergies it would be good to list those. Some people bring in a paper with all of their medical and surgical history as well. Glad you can fit all of yours on a business size card. Some of my patients fill an entire sheet of paper.

I also keep a typed printed list with all meds and Doctor who prescribed, allergies, appointments, notes from appointments or questions need to ask, list of vaccines, flu shot. X-ray’s, Lab’s and location Pharmacy address and phone. I update on ipad for each appointment then print I also keep printed copy in my purse .
That's a very thorough and complete list of your medical records.The only problem is I would have to have a small book to list it all. I do however make sure I write down questions I have to ask my doctor before I go to the visit. Thanks so much for the input. Fancy 59.
I’ve always done this for family members I cared for & myself. The only difference I did a full sheet on 1 side. Must have date of birth. If you’re in & out of hospitals, they like to know the time of day you take each med. Doesn’t hurt to have Pharmacy info. Make several copies, they can keep it in your chart or easily scan into their system & return it. Also date it every time you update & print it. That’s for you as well.
