I have the same Gleason numbers. I am 62 and was diagnosed last July. My entire prostate was covered with tumor and although the bone scan and CT scans were negative, they assume it metastisized on a cellular level, but there is now way to prove or disprove it.

I did 6 weeks of external beam radiation and also had the HDR (Temprorary brachy) surgery back in October. My PSA went from 20 to <.01 over about 4 months. (It got as high as 29, but I was able to bring it down to 20 with diet, weight loss and supplements before starting medical treatments). I am doing the hormone suppression therapy with quarterly Lutron injections and daily taking Zytiga and Prednisone, potassium supplements and a whole protocol of supplements and a healthy diet to try and kill off any cells in their infancy while they are weakened.

If your tumor(s) are smaller than mine, there are other treatment options like cryoablation that cold b helpful, but I was too far gone for that.
