Balantis? Urethritis?

Hi, guys! I am new here! I've had urethritis not long ago and I followed a treatment.My penis glans are redish most of the time, with some discomfort, feeling like I am about to pass urine and that I will not be able to stop it.Currently I am using some antibiotic creams for this, not getting much help from them. This feeling gets better as time passes after I urinate. When I urinate or after I ejaculate, my penis glans get really really red, my ferulum gets red and looks like a cut( I am not sure if it actually a cut, never passed blood). Peeing isn't a chore as it was while I had urethritis, but when I am finished, I am not certain I pass everything, so I keep pushing , always being able to pee a bit. I don't know if the pee is in the urethra , or if it comes from the bladder as I am not able to eliminate all the pee. I'll stop masturbating from now on and see if it has any effects. Also feels like I have some pee left in the urethra, but I never had discharge, even tho I always feel like I won't be able to stop it and that it will eventually get out. What do you think it is? The redness of my penis glans, which are less observable as time passes after I urinate, and come back strong when I do?

Consultant urologist, may be you need to take antibiotics. Possibly there is a sore in your urethra pipe

I'd also recommend checking w/ a urologist or your GP at the very least. When I was dealing w/ balanitis, anti-fungal creme was recommended rather than antibiotic creme.

Hi Arthur, Im dealing with the same thing, was their something that started this for you? What have you tried so far?

I don't know what started all of this. I've taken doxycycline for a week alongside flamexin(powder) for the urethritis. Now I use triamcinolone as cream. Before using this cream I used clotrimazole cream for about 2/3 weeks, but I haven't seen any progress.

Hi Arthur, you might still have an infection there go back to your GP or urologist it could be coming from further up your urethra. get tested for UTI it can take a while to clear an infection and different antibiotics. I had a UTI for months before it finally cleared.

MrMac99 - Whoa! A UTI that persisted for months?!?! That sounds rough - you have my sympathies.

Yeah it was a rough year on and off started with UTI and developed into prostatitis so it wouldn't clear the first month was the worst and all the tests. I was like Tom Hanks in the film The Green Mile. but I'm ok now haven't had any bad symptoms since Christmas. 🤞

The last antibiotic was cefalexin but I'm not sure if it completely cleared My symptoms I still had some symptoms after but maybe it was from inflammation that I had. I think it eventually just burnt itself out.

If you still have symptoms you should get them checked out with your GP or sexual health clinic.

If you have a discharge and you have problems starting or stopping your urine you should get it checked.

Also , I don't know if this is normal but I observed by watching in the opening of the urethra that the walls of the urethra are covered by some substance. I don't know for sure what texture it has , of water or sperm. Noticed that I have some mucus in urine I thought I might have some sperm leakage but it might be the substance that I've just talked about.

How was your urethritis diagnosed ? sexual health clinic ? if you still have these symptoms, you should return to them for repeat assessment and intraurethral microscopy.

The doctor looked at my penis glans, under my frenulum I think I don't know he did a move I wasn't looking at what he was doing.

Ok. So here urethritis is diagnosed by taking a small swab from inside the urethra ( water passage ) and counting the number of inflammatory cells . Based on this we diagnose "urethritis " and treat. There are different antibiotic courses depending on the persons symptoms. I presume you had a negative chlamydia test ?

I had a cystoscopy which showed ‘irritation’ in the urethera and was told it would go away it would go away itself.. It hasn’t gone away...

Symptoms include a constant feeling of irritation in my Urethera, it feels worse after urinating but is always there.

I also have a very dull reddish patch on my glans penis around my pee hole and down the front of the penis head. (I have a post in this forum with a picture)

I have seen many doctors and done many tests which are all negative.

No stds, nothing showing up on swabs, urine tests, blood tests.

Have had symptoms for over a year, all started after having sex with a girl.

Have tried fluconozole, doxycycline, augmentim, Trimethoprim and lots of different creams etc.

Also did a full yeast detox.

Haven’t been able to figure it out yet.

I am on amitripdalne currently for the last few months and trying the do nothing approach.

Any ideas or suggestions would be very appreciated.

In a sexual health clinic setting, we would take a sample from inside the water passage , and check for microscopic evidence of urethritis. If we found that , then we follow national guidelines to manage it. First line treatment is doxycycline 100mgs twice daily for one week. If you have access to mycoplasma genitalium testing, we would do that. This sometimes requires an extended or alternative form of treatment, like moxifloxacin. We would only treat with antibiotics if there was microscopically proven urethritis. Not all problems are due to infections.

Hi Spartacus. For your penis symptoms ( irritation in your urethra ) you need to follow the advice I gave above if possible . I saw the pic you posted, there is some mild redness on the skin. I don't think it is any yeast infection at all. I would try a mild steroid like hydrocortisone 1% twice daily for a month and see if this improves it.

I have tried a few mild steroid creams but can give it another go for a longer period like a month.

I have also started again on doxycycline, 2 per day for 7 days as you suggested.

Will keep you posted.

How do I request a sample from inside the water passage? Do you mean a biopsy or a swab?

A swab, an intraurethral swab. This is usually only done by sexual health clinics. There is no point in doing this at present if you are taking doxycycline. If your symptoms persist then you need to seek specialist sexual health advice , or follow the testing I mentioned above for mycoplasma etc if available in your country.

No change with the doxycycline.

I had my urine tested for Mycoplasma and tested negative.

Is it worth teying the antibiotics for this in any case?

I had a number of swabs taken from in my urethera a while back and all came back clean.

Are they worth redoing?

The sexual advise specialists (2 opinions) were of the opinion that there was nothing wrong with me and suggested amitryptaline and stop using all creams. I have done this bit the feeling remains.

What would you suggest next if I were your patient?

It seems like all the doctors in my country have dismissed what I am feeling or have passed me on to someone else.

Again, really appreciate your opinion and advice.

If the swabs had microscopy ( looking down the microscope ) as opposed to growing bacteria ( culture ) which was negative I would be happy with that. If you have had all of these tests done, with continuing unexplained symptoms, at this point I would try to focus on what improves your symptoms, rather than endless investigations. Im afraid there is not much more I can suggest.

Hi Properdoc, latest swabs showed gram positive cocci present in the urethera.. what does this mean? What antibiotics should I try given what I have tried already?

To be honest, this could be entirely normal. These could be staph aureus or streptococci - both of which can live on the skin in people with no symptoms. The antibiotics you have had already, should have more than covered for this.

Hey guys!I am feeling much better , but I have some new minor problems .My penis tip is redish, not as red as I used to have it and sometimes when I pee I have a burning sensation , and , other times (I really don't know what causes it) I have my penis tip looking wet,I could even say it's shiny(looks like some discharge,but it never makes it out, it sits on my penis tip), but it's not urine, and if I try to take it away it is not working(it is like some discharge I think I really have no idea). One more thing , that for some time made me think that this is all the cause of the problem, when I pull back my foreskin I've realised that my frenulum looks a bit detached on one side, and it looks tighter on one side and it hurts when I touch it on that side that looks tighter, what could this be, a torn frenulum. Hope you get to see this message and I hope you have time to reply! Thanks in advance!
