It looks that you have phimosis. I think you should get circumcised.
I also think it looks a lot like phimosis, you should see your doctor and see if you could get some steroid creams and try stretching the foreskin to loosen it, you should look on the internet how to do it properly because it’s quite hard to get right apparently, the steroid cream will help with inflammation from the stretching. If your not able to stretch properly or you need a fast option you could think about circumcision but research it first to make sure it’s right for you.
Steroid cream ain’t gonna help severe phimosis like that. And you may have a yeast infection if you haven’t been able to clean behind the head. Book and appointment with your doctor asap.

Stretching has a really low success rate and takes a really long time. However it may work when the phimosis isn’t that bad, but in cases like this I think a circumcision is required but I’m no doctor.
When not hard i can pull back all the way without a problem and when hard i can pull back but not a the way over tip of my penis
Yeah I was the same man. In a hot bath I could pull mine back all the way but when hard it would not pull back
Please don’t brush off this issue as it will become a problem when you become sexually active (if your not already). It’s better to get it sorted out as soon as possible.

The reason the success rate is low is because 1) people don’t give it the time it needs 2) they are stretching too much or too little 3) they get tired of waiting and decide to get circumcised instead of finishing the process. But it will work if you give it time do it properly and don’t give up, but I’m not saying you need to stretch and can’t have a circumcision if you want, but I’m am going to warn you circumcision is a permanent solution and you should make sure you need it and that you want it, and you are will to take the risk something will go wrong because, it is a operation and things do go wrong.

How does stretching work and for how long should I try doing it
Stretching works by stimulating the foreskin because the skin is undergoing too much stress so this is how the body takes action, but if you stretch to hard the foreskin can become inflamed and this will hinder the process of stretching and the foreskin won’t grow. So the steroid cream helps prevent inflammation and allows the skin to grow more efficiently.
The time it takes to stretch the skin can vary a lot depending on how bad the phimosis is although should not take much longer then 6 months maximum, that’s one of the reasons people opt for circumcision.
Try out the stretching and see how it goes.
However if you do go ahead with a circumcision make sure you do your research. Ask your doc what style he does etc. Or even better I’ve heard that plastic surgeons do a better job at circumcisions but it’s a little pricey

Well if you want to spend 6months to a year stretching out your foreskin every day for 5min three times a day, that’s completely up to the individual and in most cases people give up because it takes too long.
While it may be true some are too impatient, it’s also true they aren’t given options, nor are they informed of the consequences of losing their foreskins.
Stretching takes no more effort than masturbation, and can be incorporated into thevpractice.
I think some of the respondents have missed the point here. Is this something fairly new? I mean, were you able to masturbate painlessly in the past? If so, thin I'd say you have a simple inflammation. I'm not a medical professional, but I'd guess there's no need to jump to any sort of circumcision conclusions. Lots of people have mild and totally harmless phimosis, although I noticed it's quite rare among women. (Don't get all weird, I'm Joking.) But if this problem is something new, it's probably just something that a doctor can prescribe a cream or antibiotic to resolve.
