So a few hours ago I noticed that my foreskin was a little bit pulled back and it looks like letter D should I be concerned or not

It’s nothing to worry about. Put your index fingers in the opening and gently but firmly pull it open. Hold five minutes. Do it for times per day. This will enlarge the opening to allow full retraction.
