I'm an uncircumcised male in my late thirties. About a year ago I noticed a red spot on the head of my penis under the foreskin, on the lower left side next to the frenulum. The spot is about 1cm in diameter, or a little bigger. I didn't think anything of it because it wasn't painful and I got tested for STDs and was all negative. In July I contracted syphilis and developed a lesion on the inside of the foreskin in that same area. I got treated with three shots of penicillin over several weeks and it did heal that lesion, and in a recent blood test the traces of syphilis were almost gone. But since July that initial red spot seems to be worse, often very red, especially after sexual activity, and it sometimes looks wet or shiny. It doesn't hurt but can sometimes feel uncomfortable, like a slight burning sensation. I've seen a urologist and he prescribed a steroidal cream, Clotrimazole/Betamethasone. It seems to help temporarily but does not solve the problem, as the redness still returns after sex. About a month ago I had a biopsy done on the area and it came back negative for cancer, viruses, etc. So my urologist thinks it's just chronic inflammation. He suggested that a frenulectomy could help (where they cut the frenulum), or a circumcision, as my foreskin is pretty tight. Has anyone else had a similar situation? Or any advice would be so greatly appreciated. Thanks in advance.

Maybe a frenulectomy would be helpful.Also try stretching your foreskin to make it looser.
Circumcision should be a last resort. I would explore other options and get another medical opinion.

STDs and inflammation live only in moisture. The therapeutic effect of circumcision is to remove the moist environment on the penis. Frenulectomy only relaxes the frenulum. It has no effect on infections and inflammation. I had a frenulectomy. The foreskin was looser, but also inflammation more often. After release, the foreskin hung more under the glans and more urine remained, more moist. Only shortening the foreskin and dryness on the glans will solve your problem forever. I found out this truth after 3 years ! , when I had many problems similar to yours. It is not necessary to remove the all foreskin during circumcision. Just shorten the skin so that the glans remains completely exposed in the underwear.
