We cannot diagnose health problems on the forum. You will need to get it checked out by a doctor.
What I would do is find a urologist and share the pic with them. They would be able to evaluate it properly when seeing it in person.
I started to get blisters on my foreskin and decided that after years of one issue or another, it's time to get circumcised. It would solve a lot of problem for me, so I went for it. Doc did not advise against it.
