Heparin has a rated 12 hour half life so missing the dose before the procedure should be all that is required. As others have said consult with your doc for confirmation

Hi Judes

When I had cardic bypass surgery I was on Warfarin, which i stopped a week before surgery and started Fragmin the same day that I stopped Warfarin. On the day of surgery I missed my morning Fragmin shot, as the surgeon said that they would be using IV Heparin during the operation. I re-started Fragmin shortly after coming round in ICU and started Warfarin the next day; keeping the Fragmin going until my INR was back in range.
