Good Afternoon

Is any able to explain to me other than APS is their any reason why I should have a below range in immunoglobulin G,A,M

The reason I am asking is that I am usually seen at St Thomas’s Hospital who I have excellent care with

Now following a blood test from a local surgery ( who I don’t think a lot of due to past experience ) I am being sent to another hospital as well, I have phoned the Doctors and they are going to phone me on Tuesday the appointment is a week before I am seen in London thanks any thoughts would. Be appreciated

In autoimmune diseases, the immunoglobulins can be abnormal, mine are and have been for many years.

You need to see a haematologist, who may want further tests.

Thank you, that’s what I thought, I already see a Haematologist in London who is excellent,

The Gp phoned me back and when I asked he said it was above his expertise, about time they had more training then, I thought anyway looks like I have to wait until Tuesday to speak to the Dr who booked the blood test

Really pleased you already have the appointment on the near horizon, don't forget to jot down any questions so you remember what to ask on the day, this is what I do. As lupus-support1 mentions, it is not uncommon with autoimmune disease and certainly shows up with Thyroid disease. MaryF

There may be a difference in the assays used and value ranges between labs. So you may want to ask your providers to check into that as well. Plus there's always lab error and fluctuating antibody levels

Yes. These earlier replies go some way to explaining this can have several causes.

e.g. Here’s the relevant bit of my story (am 68, officially diagnosed with sero neg infant onset lupus …+ some signs of possible APS (undiagnosed):

Years ago when my Univ hospital rheumatologists ran BonePanel Blood tests inc immunoglobulins before starting IV Zoledronate osteoporosis treatment, the results showed below normal ref range IgG, A & M.

We already knew from years of blood monitoring that I had chronic lymphopenia, but, over the years, only my dermatologist at our smaller local hospital had bothered to run immunoglobulin G,A,M tests, which had tended to result below normal ref range. She’d written to my rheumy to make sure these signs of immunoglobulin deficiencies were documented at my Univ Hosp. But my rheumy felt they weren’t yet far enough below normal ref range to take action - even though my records also documented that I’d spent my whole life suffering from what I now know are termed: pockets of chronic multisystem persistence pattern bacteria driven sepsis.

Anyway, when my rheumy’s pre IV Zol test results came in, she immediately put me on an urgent pathway for further more sophisticated blood & urine tests + urgent referral to immunology for investigations

Further investigations happened promptly & resulted in immunology diagnosing the Primary Immunodeficiency Disease (PID) panhypogammaglobulinaemia, aka Antibody Deficiency Disease, which explained my seronegativity to my official AID diagnoses (SLE, CLE, Sjogrens, Vasculitis). So Immunology has me on long term immunoglobulin replacement therapy at immunologic rates + daily long term antibiotics, which has damped my chronic multisystem infections right down!

So, that’s just 1 example of possible reasons for below normal ref range IgG,A,M. I do have several friends at my Univ hospital who attend rheumatology clinic for their AIDs + immunology clinic for their type of PID, but none have APS. And on my official U.K. PID support group forum there are a few folk with both an AID & PID. But the combo is unusual. In my case this very early onset immune dysfunction ‘May’ have been drug induced (due to effects of 5 months’ daily inutero exposure to the internationally notorious endocrine disrupting synthetic oestrogen diethylstilbestrol - scandalously prescribed to women as miscarriage prevention back in the 1950-70s )

Thank you, I had to have extra blood test and urine test too.my muscles and bones have been really Painful over the last year and I have felt really weak, really didn’t think their could be anything else wrong with me, but I suppose better to know than not.

Nuff said mine is always low. We don’t test anymore. I just need to be careful I don’t get sick.
