or, if you email Firo Action directly from their site they will email you one.

Even if you have had DLA before, get some one to fill in the form for you. i.e. Welfare rights. They are excellent, also try and get letters from you GP, consultants etc. This will really help your case as you may be aware with all these cuts they are taking thousands off DLA even if you have had it before. So my advice is get as much information as possible to go with your form to back you up on it. x

If you can get to your local Job Centre, they should be able to provide you with the forms.

If you can't go yourself, perhaps somebody could go for you.

And please get an experienced advisor to fill your form in for you.

I have used both Welfare Rights and the CAB. I'm sure you could find your local Welfare Rights using Google..
