HI lynn,
There should be a social services in your local phone book , you can phone them and ask to be put through to the ot department.
This is was what i did and they came and helped me by grab rails etc..
Sorry i havent got a social worker but you could maybe ask the social services about that too or your gp.

i phoned adult social services, explained my situation, they then sent someone out to see me and it went from there xx
if you go to your doctor and he can tell you if not refer you to an adult service that would help as my nices doctor refered he to an advecut how have done most things for her from filling forms to going to tribunels . so phone your doctor monday morning sofft hugs speek soon
You can find the numbers online to, if you go to your local councils website and look under services they'll have numbers and email's for you. I just emailed them and they called me back that day. Good luck x

I rang the same people to and they also gave my daughter a support worker and registered her as a young carer she is only 11 and they have been great. They asked OT to visit the house to and now I have perch stools they have turned my bed into ones like the hospital where you can raise the mattress and im waiting for a wet room and stair lift. They will also help you with DLA forms the lot and they have have someone who will help with tribunals
