nurse case manager

Ive been given a nurse case manager by my social worker she will hopefully review my medical history to get me the help for my varoius symptoms has anyone else had this service

WOW how did you manage that? lol

I have been Disabled and housebound for 14 years, have had little help in form of items to assist me, but after that they fade away. I was given a round handled knife & fork and it took 5 years to get the matching spoon!! I could have brought one, but it got to be me & them after a while lol
