I need some advice.. I told my boss about my diagnosis..exsplaining all of my symptoms and issues that occurred or are likely to occur...I have never let her down have gone to work every day even when in pain...so my issue is she has given me a supervision form to fill in about my performance at work I'm only part time I might add..also only work when school is open.(term time).... but since telling her about my illness...she keeps dropping snide remarks about is my medication affecting me am I ok to work because I have strapped up my hands or various joints just to get me through the day... but the comments I find unnecessary... but I want to put this onto my form without it becoming a issue and let her think she can comment at me in front of people...but want to at the same time nip it in the bud as to speak.... and to highlight to her how she is making me feel.... what do I put so I don't offend to much.... any suggestions

I would carefully explain that your diagnosis and the fact that you disclosed it should remain confidential between the two of you and not open to discussion when others are present, you could also say that you appreciate her concern, so as not to look confrontational xx
I agree with Lou1054 and carefully explain how you are feeling and the confidentiality that you expect in the workplace. FMA UK do have some printable booklets and they may help you with this? I have pasted a link below:
If it were me, I would go confidentially to Human Resources and tell them what is happening. Make sure they know the conversation is confidential.. Then ask their advice... Good luck - It's a hard place to be
