I am so genuinely sorry to read this my friend and the only tip I can offer is to pace yourself which does not really help where work is concerned. The Occupational Health Assessment could prove quite useful for you as they will make recommendations on how to help you instead of penalising you? I have been through similar, and the first thing I would say would be to have a look at 'The Equality Act' (It used to be The Disability Discrimination Act). There is a section about employment and that they must make all reasonable adjustments for you to be able to continue in your job. This could be specialist equipment, such as a particular chair, desk or equipment or even a change in your working hours, such as reducing to part time work, or changing hours (if possible).
If this is applicable, you are able to take your trade union representative with you to all grievance meetings, but your employer should inform you of this anyway.

They visit you at work and make recommendations to your employer about what equipment you need.
Also I find meditation and swimming helps my stress levels. I use the headspace app for guided meditation.

first, are you in a Union? They can help; also ACAS if you fear for your job. If you have held this job for more than 2 years they cannot just sack you, and they Must make reasonable adjustments to allow you to work.
It can be like jumping through hoops, but stay strong; if you quit you will be barred from JSA for a certain amount of time.
Read up on the Disability Discrimination Act of 1995 and the Equalities Act of 2010 for information on your rights.
Going through occy health assessment at the end of the month - whoopee how exciting (not). They tried to get me to sign that I would release my medical records - I refused. Reasonable adjustments at work does not need full access to my records. I supplied only my diagnosis letter. They are visiting the work place for an hour to watch me work and discuss with me things that would make life easier. I feel it's really difficult for an assessor to make any judgements about equipment etc as it depends what sort of day I am having. There are days I can appear "normal" whatever that is and days where it will take me half an hour to get out of the car! All I can do is tell him/her how it is generally and let them make their decisions from that.
I used to work at Warwick University, I went through grievance process and finally Occupational Physician. the pension scheme gave me only partial incapacity. I appealed , were sent to an independent doctor and got full incapacity. the whole process two 2 years, but I got what I wanted and needed. keep going, do not despair. it is along process but I got it there at the end.
