Hello zaklove40 and a warm welcome to our friendly fibro forum where you can get help, support, advice and understanding.
You haven't said if you are waiting for a diagnosis of fibro or have been diagnosed for some time. For further information about fibro our mother site fmauk.org has useful guides and links, one to our benefit lady Janet who may be able to help you.
In the meantime collect all your medical evidence, blood tests etc and any clinic letters. I suggest you also make an appointment with Citizens Advice
Perhaps you could tell us a bit more about yourself and your situation.

Best advice - get advice when filling forms! CAB good place to start. Take your time and provide as much info. written advice as much as poss. even if your GP Surgery make a small charge. They're struggling financially but still worthwhile to get it right from the start. Really hope you get a fair assessment. xx

I agree with Bluebell and Tulips.
You can submit photocopies of any appointment letters you still have. GP needs to be aware you are applying for PIP as their details go on the form and any other healthcare professionals. I also would add there is a site called benefits and work which helped me get the right sort of info on form. The key words are can you..... reliably and safely!
Meals prep no chance as I have accidently burnt myself or cut a nick from a finger. Lost count of how many pans I have ruined with overcooked food.

Start gathering your evidence now even before you get any forms as sometimes it may take a while. Like skit I used the Benefit and Works site as it was well worth the subscription as they have step to step guides on how it is best to fill in the forms. If you can get someone who is experienced at filling in these particular forms like the CAB please do so, again ring the CAB up asap as sometimes their waiting lists for an appointment are long. Start keeping a diary daily on how you feel ,how much pain you are in how long it takes you to do daily tasks and what help you need
Any aids you need to help you even if its something like holding onto things to keep your balance when getting dressed .
It will help you complete application and you can send in diary with application .
If you are unable to get help filling in form just read questions carefully and focuses on what they are asking .
