If you can get some professional help from an organisation like the CAB do so. If you go on their website they have a step by step guide. Also Janet the Fibromyalgia Action UK advisor will I am sure be able to help you as she has access to the benefit and works guide her contact details are in one of the pinned posts to the right of your screen.
Remember it is not so much why illnesses you have but how they affect you I everyday life especially working life. If your GP etc have intimated that they feel your illnessess will affect you long-term do remember there is also the PIP benefit.
If there is one question you are really stuck with I am sure members of the forum can put theirs heads together to help. Good luck.x
Google the Benefits & Work site. Inexpensive to join and download excellent guides.
The benefit and work website seems to be the option now (sorry i dont know how to post a link) as most people come back and say they cant get an appointment with CAB in time.
Blessings

I sincerely hope that you are feeling as well as you possibly can be today? Welcome to the forum and it is wonderful to make your acquaintance. I have pasted for you below a link to our mother site, FMA UK which hosts loads of useful Fibro information:
I would definitely get some professional help with the form filling, the CAB are usually very good if you can get an appointment? I want to sincerely wish you all the best of luck and please take care of yourself my friend.
All my hopes and dreams for you
