Scanning the site, I see many people are talking about having to claim JSA whilst awaiting Mandatory Reconsideration or after losing their appeal for ESA. Here details to a template you can declare your disabilities, how it affects you & what reasonable adjustments you would need. This was originally written by Ipswich Citizen Advice Bureau (CAB) to alert Job Centre Plus of your disability when claiming Jobs Seeker's Allowance (JSA) to try to avoid what appeared to be unfair sanctions put onto disabled people.

Just read your post, I have been thrown off ESA by a very rude woman. Anyway I have had to sign on JSA and was unable to go when I had to sign on (was on morphine for psiatica. You may find this bit interesting because it's happening every where. I gave my mandatory reconsideration to my JSA adviser, as per telephone instruction from ESA, she in turn was supposed to fax it to Hackney our regional office, this was two months ago. On Friday I found out from head office in Liverpool that it was never faxed and I have to reapply, my advicer has been aggressive when I have mentioned it??? She also told me over the phone that because of my sick note I will not have to physically sign on .....I have however been sanctioned and am receiving nothing. Please tell everyone to send there mandatory reconsiderations by post (recorded) and don't rely on people's word, clearly I could kick myself now but def won't make that mistake again. I am so determined now that I have recruited a small army, so I can take witnesses with me and am also recording all calls even if they won't be admissible should we go to court?? But at least I can prove it to myself that I'm not losing the plot coz it kinda feels that way sometimes. Good luck and hope your having a good day all the best xveronicax
