Hi Guys. Reading the horror stories that is applying for PIP, i'm due for assessment April 2015. Ahead of time my GP has issued a Care Plan which is useful when coming into contact with out of hours health 'care' lol. It details my diagnoses, meds and physical difficulties (I am housebound and have many problems associated with FM & some). It may not be possible for everyone, but I am hopeful that it will help come April 2015. If you have a good relationship with your gp it must be worth a try! Tulip xx

I think this is a very good thing and should be made standard practice for long term conditions, save a lot of nonsense when seeing someone new. Lou xx

I sincerely hope that you are feeling as well as you possibly can can be today? Thank you so much for sharing this with us all. I have a care plan related to my Asthma and COPD, which is meant for out of hours care and reduced hospital admissions. Mind you, I cannot remember the last time I had such an attack that I needed hospitalisation? It is very good and useful to have however, as it may come in handy one day.

Thank you Ken, you are always so lovely. Am I allowed to say that? Well anyway, you are. I also have a lung condition and on 24/7 oxygen concentrator. Million other probs. and it is somewhat worrying when medics look at you with such expressions of horror! I've done well lately in reducing opiod pain relief 'cause of course it affects the lungs. After everything I've heard lately, wild horses wouldn't get me to hospital premises. Full (REALLY full) of very sick people. So sad. Too scary. Def. much better in my own home.

I think you will find that the care plan is now standard nationwide and is offered to those with long term problems and is of course very very good in that all your information is readily available to other who are in a position to offer medical treatments if you are away from your regular surgery and will prevent any delay of your treatment if you are seen by any one other than your normal GP !
