After looking on the Citizen Advice Bureau (CAB), they give this information on the penultimate page of their ESA diary document;
Contact the Jobcentre Plus office you have been dealing with if you are unhappy with the service you have received - their details will be at the top of any letters they have sent you. You will need to explain what has happened, how this has affected them and what they want to happen to put things right. Jobcentre Plus will try to resolve the issue over the phone or deal with the complaint within 15 working days.
If the complaint is not resolved, you will be asked if you want your complaint sent to the Director General of Operations for the Department for Work and Pensions. They aim to deal with complaints within 15 working days.
If you are still unhappy, you can then ask the Independent Case Examiner (ind-case-exam.org.uk/) to investigate - they’ll be impartial and this is free.
If you are unhappy with the response from the Independent Case Examiner, you can ask your MP to send the complaint to the Parliamentary and Health Service Ombudsman (ombudsman.org.uk/) (CAB)

Mine was about 2yrs ago is it too late to complain. I had failed the assessment so got taken off wag and the judge got me really confused by asking questions based on my previous claim. She had already made her mind up I had failed and was determined to try and catch me out. I appealed to upper tribunal with the cab but didn't feel the paperwork submitted by them fully explained my reasons for complaining and the upper tribunal supported the judges decision. I had another assessment earlier this year where I was put straight into support group so a good outcome eventually but needless to say I did not have a miraculous recovery between the 1st tribunal who awarded me 18points and put me in wag and my last claim!!
