you should get a letter from your employer when sick pay ends informing you that you need to apply for ESA after sick pay ends. if you don't get a letter then go to the benefits.gov.uk site and it'll tell you how to apply. Good Luck!

When your Statutory Sick Pay runs out your employer should give you an SSP1 form, indicating the end date. Hold on to that since you will then need to make a claim for ESA, as indicated above. You can do this on the New Claims phone line 0800 055 66 88. They will take you through questions and the full statement will be sent out to you for you to confirm, sign and return for processing in the envelope provided. At this point they will ask you to provide an SSP1, (and other documents, one of which will be a sick note from your doctor confirming you are still unable to work.) so send it in with the statement (take a photocopy first for your records, but they will need the original). Hope this helps and good luck!
